Ee picked up a charged fire hose and the nozzle came off causing high pressure water to hit ee's eyes. Seen at ER and diagnosed with contusion to eye, released to full duty. On follow-up on 04/15, ee was given a prescription for eye drops.

EE was lifting a heavy object when they felt a pop in the right calf. They reported the incident to a supervisor and worked out the shift. EE was seen the next day at the clinic, given crutches, and put off work.

Employee was hooking up a trailer. The employee pulled the tongue of the trailer over to align the pintel when the trailer dropped onto the pintel the tongue smashed the employees finger between the tongue and the block the trailer had been setting on.

Employee installing chute on concrete mixer truck. Finger caught in pinch point between two chutes. Open fracture (non displaced) to right index finger.

Ee was holding a belt with both hands and watching a co-worker move weight from the belt. When the weight was moved, the belt quickly pulled the ee's hand up and it was pinched between the metal and the belt. Ee was seen at the ER, x-rays were negative for fracture, the wound was stitched and the ee released to modified duty.

EE was shoveling out a conveyor drop chute when they bumped their arm on a railing, dislodging some fine dust/lime, which blew into their eye. Seen at the ER and diagnosed with a corneal abrasion. Follow-up with eye doctor diagnosed inflammation and irritation, prescription eye drops given. Released to full duty.

Ee was pulling a pump spool and got some hot slurry on the left side of face, neck and left arm, and strained left leg. Further exam on leg on 7/19/18 revealed a torn hamstring and the ee was put off work. Referred to orthopedist.

Ee finished loading a round and when the went to remove the remote from mine belt, finger contacted open box cutter causing a cut. Stitches required to close the wound. Ee released to full duty on next scheduled shift.

Ee repairing broken feed change. Operator rotated the feed motor, chain moved and pulled the ee's hand, hydraulic driven clamp closed, crushing hand in bolter. Ee seen at ER, diagnosed with fracture to right finger, released to restricted duty. Site could not accommodate so put off work.

While tripping out rods, the employee was moving the AQ spacer pipe from on top of the drill pipe to the storage area under the drill pipe. The AQ rod missed the inner most cross member and levered up pinching the employee's hand between the AQ and the drill pipe. Employee's middle finger was lacerated requiring 4 stitches.

Ee was removing hydraulic cylinder, the cylinder slipped, finger crushed. Seen at ER and diagnosed with a crush injury and referred to an orthopedist. Orthopedist confirmed fracture and released ee with restrictions that site cannot accommodate.

Ee was moving a VSD on rollers. When going into the MCC, the VSD was pushed sideways into a cement wall, crushing the ee's left index finger. No fracture, but stitches required to close the wound. Ee released to restricted duty.

Crew was moving a temp electrical panel for crane access during a clean-up of a jobsite. While sliding the temporary panel along the grating, panel base became lodged on a cover on the grating. EE stepped in to help the crew, placing hand between two objects. As the crew maneuvered the panel, it jumped and the injured EE's hand became caught between the temp panel and a pipe.

A laborer was tending a bricklayer in an autoclave. The laborer reached into the bucket to retrieve a brick that had been saw cut. The brick cut through the EE's leather and latex gloves. The EE was transported to the hospital where EE received 8 stitches to the left index finger.

While rolling 24 inch casing on a staging rack to the crane the employee pinched finger between the end of the wrench handle and another piece of casing.

EE mucking when a bolt dropped. EE got out of mucker to pick it up, and an electrical plug struck EE in the elbow. Seen at ER, diagnosed with contusion and small chip fracture, referred to ortho.

While tripping out tooling, staging the SXD tongs onto the drill collar, the tongs moved to the left resulting in a laceration to the left middle finger, requiring stitches.

Ee was lifting filter out of tub, leaned in and later felt pain in chest and abdomen area. Was not sure related to work. Sought treatment on 10/24/16, diagnosed with contusion, returned to work on restricted duty. After investigation by site and workers' comp, determined compensable on 11/14/16.

EE was cleaning sump pump, lifting buckets of material, and felt sharp pain in left shoulder. Seen at the clinic and diagnosed with acute strain to lt shoulder. EE seen at clinic on follow-up 10/17/16, placed on restricted duty and prescribed physical therapy.

Ee was pulling on a hose and hit elbow on an I beam. EE was seen at the clinic and fluid was drained from the elbow and a steroid injection given. Ee put off work until 10/10/16.

EE was resting a pipe on shoulder. Once clamp was placed on pipe by coworker, EE moved away and felt a sharp pain in back. EE was seen at the ER and give meds for pain. Follow up resulted in investigation of compensability. Accepted as work related injury on 10/03/16. EE off work.

Team Member lifting a grate lost hand position and finger was caught in pinch point.

Ee was lifting a pump motor to see how heavy it was before loading it in a truck. EE felt pain and a burning sensation in the left groin area. EE was seen at clinic and referred to a surgeon for possible hernia. Exam with surgeon was negative for a hernia, diagnosis: groin strain. Released to restricted duty.

EE smashed pinky finger on left hand while rebuilding pump. Diagnosis laceration, released to restricted duty.

Ee was moving a jack stand when the upper part of the stand fell, striking the right middle finger. Stitches were required to close the wound, and ee placed on restricted duty which was accommodated by the site.

Ee lifted a ladder and was putting it on a cart; ladder shifted forward, ee reached and twisted to maintain control of the ladder and strained back. Saw doctor on 05/02/16 and was put off work pending follow-up, making this reportable. Ee remains off work.

Ee raising hood on carbon truck, when EE stepped down EE felt pain in EE's lower back. Pain got worse as shift went on, ee reported to EE's supervisor. Worked remainder of EE's shifts and went to Idaho for days off. When EE returned to Nevada on April 6 EE saw doctor and filed a workers' comp claim. EE was placed on restrictions that the site could not accommodate.

Ee was pulling a pan on casters, and strained EE's back. EE was seen at the ER and diagnosed with a lumbar strain. EE was put off work for 3 days and follow up resulted in a modified duty release.

Ee was removing a barrel from a pump, and had a chain fall attached to it. The barrel broke free and pulled the ee's right hand into the intake pipe coming from the flash discharge tank. The right thumb was severed at the distal knuckle.

The employee was throwing trash in a dumpster when EE smashed EE's finger. EE was seen at the clinic and diagnosed with a fracture to the tip of the left ring finger. The wound was stitched and the finger placed in a splint. Ee was released to restricted duty.

Ee was lifting a hose in order to connect it to a tank and strained EE's back. EE went to the clinic on 03/03/16 and reported EE had a work related injury. EE was seen by a doctor and placed on restricted duty that the site could not accommodate.

Ee was using a buffer on the inside of a tire and it came loose. Ee grabbed buffer and pulled back, cutting the right upper knee. Ee was seen at the ER and the laceration was closed with stitches. Ee released to full duty.

Employee was lifting a NKM screen. She noticed the screen was caught up and tried to maneuver it, the screen pinched her hand between a rail and the screen causing a fracture to her rt. little finger.

EE loading guide support parts on deck, pinched finger between guide support piece and deck. Laceration to right ring finger, stitches needed to close wound. Released to full duty.

Ee was fixing shotcrete equipment, pressure released and employee was struck in face and chest by shotcrete. He was seen at the ER and diagnosed with chemical burns in both eyes. He was referred to an eye doctor for further treatment. Put off work.

Ee was closing hatch on a mixer truck when some shotcrete flew up into his eyes. He was seen at the ER, diagnosed with a corneal abrasion, and prescribed antibiotic medication. He was released to work with no driving for two days and then full duty.

Aligning Membrane on boiler tubes, pad eye weld failed, pad eye struck employee in right knee cap.

Ee troubleshooting leak on torque converter of mucker when A/C unit fell, pinning him. His coworker removed the unit and the ee was seen by a doctor for left arm, shoulder and upper torso pain. He was released to full duty. On 06/24/15, MRI results revealed a labrum tear and the ee was released to restricted duty pending surgery, making this a reportable injury.

Ee was removing intake pipe on a 930e Haul Truck. The pipe fell and pinched the ee's finger between the pipe and the frame. Seen at ER, diagnosed with a fracture and laceration to the tip of the left index finger. Released to restricted duty.

2 EEs were carrying a heater, load shifted and strained 1 EEs mid-back. WorkCare called, advised self-care. Incident was reported 1 shift after occurrence. 5.8.2015 Dr released EE w/care not to aggravate back due to findings of Xray & MRI as soft tissue injury. 5.11.2015 F/U appt EE was again release care not to aggravate back. F/U on 5.18.2015 EE was given restriction. F/U 6.1.15.

On April 16, 2015, while employee was lifting a generator (with two other employees) he felt a pain in his abdomen. He continued to work without any complaints and declined medical treatment until May 13, 2015. His first medical appointment was on May 19, 2015 at which time he was diagnosed with a hernia that was surgically repaired on May 27, 2015.

After tripping out, locked bit in centralizer and spun bit off rod. Noticed that the pin end of the rod was braking off and went to check it. Didn't notice that rod was barely hanging onto the striker bar. Stuck finger under the rod to check and the rod fell off the striker bar and pinned the finger between the rod and the bit that was in the centralizer.

Ee was fixing a leak in the paste plant and got shotcrete in his boots. He noticed that his feet and lower legs started burning. He was seen at the hospital and diagnosed with 1st and 2nd degree burns on both feet and lower legs. He was put off work and is attending physical therapy.

Ee was climbing a ladder onto a loader when he felt a pop in his right calf and had immediate pain. Seen at ER, referred to orthopedist who placed him on sedentary duty with a boot and crutches. Site unable to accommodate sedentary duty.

Ee was reeling up a hose reel and it came loose and fell on the employee's hand. Fractured 4th finger on right hand. Surgery required. Ee put off work.

Injured EE was clearing up the job site & was placing metal refuse into an open trailer when a piece of metal refuse dislodged from the refuse pile, came down & struck EE's left forearm above his glove. His left forearm was lacerated. First aid was administered at the job site & he was transported to the hospital. Prognosis after surgery regaining full use of arm.

Ee was working with 6" pipe and his thumb was caught between the pipe and boom. He was seen at the clinic, diagnosed with a closed fracture and a laceration that required stitches. He was released to restricted duty.

While rotating sheave, employee pinched his right thumb between the sheave and the belt.

Ee was washing off powder truck, turned valve the wrong way and lost control of the hose which struck him in the head. He received staples to close the wound. Released to full duty.

Ee was pulling a hose off a reel for fueling, turned and felt a twinge in his back. He continued to work and as the day went on, felt more pain and reduced range of motion. He was seen by a chiropractor and has been off work receiving treatment.
