Miner was sitting on apron of screen before the Roto-Lift chipping clay out of fins while screen deck was not operating, but still energized. Roto-Lift started turning while miners feet were between fins, causing left leg to become crushed between Roto-Lift fin and frame of screen. EMS was called and miner transported to the ER.

Operator had just returned to work from a double knee replacement and the impact on the knee caused the incision/scar area to reopen.
Employee was attempting to remove an insert out of EE's drill steel. EE was beating out EE's steel when EE felt a pop in EE's right elbow. Doctor's restrictions took EE off work on 5/27/2026.
While an employee was manually turning the HPGR drive with a prybar the inertia of motion from the HPGR rolls continued to shift the drive with the prybar wedged in between while the employee's hand was still grasped onto the prybar. The force of prybar pinched the employee's right pinky finger against the bottom of the metal cage used to guard the drive when it is energized.
A roof fall occurred underground at xc 23 number 5 entry on the e. mains.
While loading 14-foot jumbo steels onto the rack, the jumbo began reversing; a steel caught on rock, sprang free, and struck the IP's left knee.

Lost incoming power from Rocky Mountain Power. Mine fans were lost and mine was evacuated. Notified MSHA. Power was restored at 7:00pm and miners returned to the mine by 10:30pm.

while changing teeth and cutting edges on 988H loader 23.3804 eq #. a wrong bolt was inserted in one of the holes by mistake and while removing it EE cut EE's left ring finger. first aid was rendered and later in the morning went to Urgent Care where they cleaned the wound and did 4 stitches. no restrictions and returned to work same day
Employee received abnormal results on hearing test for right and left ear. Employee was retested and for a second time received abnormal results for right and left ear.
Exposure to Coal Mine Dust. Individual hired at Buchanan Mine #1 on 4/14/2026. Individuals Radiologist interpretation was concluded on 4/21/2026.
At approximately 10:50 PM, two employees were cleaning beneath the screw conveyor when one employee came into contact with the other employee's tool, resulting in an injury to the affected employee's right eye.
Employee was on a safe rack when the flooring gave way. Employee did not fall all the way through and fall protection was in use. During the process of catching self the employee bruised the left arm and left leg.
Miner was taking the pin retainer cap off of an excavator bucket (heavy round collar weighing about 30-40lbs). EE had to remove three bolts holding the collar on. It released easier than expected. EE's fingers were crushed between the collar and the bucket, breaking EE's left ring, and middle, fingers.
Employee was repairing a Raymond Roller Mill Journal assembly. EE was lifting the part with a sling and forklift to position it on a work table. EE attempted to reposition the part before placing on the table when the assembly came loose and a portion fell crushing EE's right thumb.
EE was using Metabo Grinder in an inverted position and cutting against the direction of the blades rotation. the blade kick backed, causing laceration to the left index finger.
Employee was directed to retrieve a part from another on-site location. A loader blocked the roadway, requiring travel around it on uneven terrain in a side-by-side. While driving, rough ground caused the vehicle to jerk toward the loader. The employee's left hand, holding the safety handle, was caught in a pinch point during impact, causing an open fracture to the left wrist area
Miner was cutting a conveyor belt with a utility knife when the blade slipped and lacerated EE's right knee.
The Contrary Service hoist would not run due to programming problems in the PLC Controller.
The employee met with supervisors in a parking lot on the east side of the building. After the conversation, EE turned to walk back to the office and tripped over a wheel chock (angle iron) because EE did not fully clear it. EE fell onto EE's right knee and elbow. EE was wearing steel-toe boots, and no other PPE was involved.

The driller shooter was carrying boxes into the blasting magazine. EE bumped EE's hard hat on the door frame, and when EE went to grab it, EE slipped on the magazine floor (raining and muddy). When EE's weight re-distributed to EE's right leg, EE felt it pop at the knee, which was a ruptured quadricep tendon and right thigh muscle. The knee was professionally immobilized.
Employees finger was pinched in a safety gate causing laceration.
While performing maintenance on a gearbox, a secured motor shifted after a support bolt was removed and struck the employee. The employee sustained a neck injury.
Employee was walking into the maintenance office when EE tripped, fell and land on the maintenance lunchroom glass door. Employees elbow/arm broke the glass on the door resulting in laceration of the tricep area.
While cleaning the cement mix tank, the injured employee attempted to clear a plugged drain hole by reaching into the cement mixture. During the process, the employee was exposed to wet cement, resulting in chemical burns.

While carrying buckets up the stairway in a yellow tub to replace buckets on EQ 250, the miner reached up to lift the tub over the railing. While turning and overextending during the lift, EE immediately felt a burning pain in EE's rib area and mid back area.
ind was stacking cable when a johnson super car was backing up struck the individual on EE's right side which resulted in a right hip fracture and bruised knee
A miner was struck in the ribs with a drill steel that was hidden under the muck in a horizontal position. When the backhoe operator raised EE's outrigger it grabbed the steel causing it to twist up out of the muck striking the miner (right side ribs).
Fabick CAT employee was working in the Cleveland Cliffs Mining Shop. The employee was working on a motor grader cylinder head, installing a fitting. The wrench slipped off and the employee's hand hit a flange, on the cylinder head, causing a small laceration on the top of EE's left hand.
While replacing an HSI crusher lift cylinder, the component shifted, pinning the employee's finger against the crusher wall. The employee sustained an open fracture of the left middle distal phalanx. Treatment included stitches, a splint, and medication with work restrictions.

While changing teeth on a 992 EE felt a couple pops in EE's left wrist.
Employee was standing next to a welder torching a bolt. Welder turned hit employee's left hand and burned left hand. Burn caused a second-degree burn. No lost time.
The Contrary Service Hoist had a roll of conveyor belt loaded on it that was attached to the 30 Ton Overhead electric Hoist that is used to unload the belt. The PC and Overhead hoist remote control had electrical problems and this interfered with the use of the Elevator.
EE was entering EE's haul truck when EE heard EE's right knee pop when sitting down into the seat. Miner had an MRI done on 5/19/2026 where a tear in EE's meniscus was found. The miner is scheduled for surgery to repair the knee.
Employee was standing next to wrecker trailer and reached up on the trailer to retrieve EE's earmuffs. When EE stepped back, EE stepped on the welder power plug connection, twisting EE's knee. EE was taken to the ER and given restrictions that cannot be accommodated at this time. Currently ONGOING.
Employee was checking for a leak in a hydraulic hose when EE received a puncture in EE's right hand index finger. EE was taken to the hospital for examination of suspected oil injection to the puncture site.
Employee bolting in #7 entry received laceration to right arm from bolt plate while installing roof support. Received 5 stitches to laceration.
I.E. Stated EE was helping put up a waffle beam across the road when the helper lost grip of the beam dropping it smashing two fingers on left hand between beam and tractor. X-Ray showed 2 broken fingers and EE received 18 stitches.
Teammate fell backwards while restacking a pallet from the conveyor
On Monday, May 18, in the first shift, the operator of the r-line 1 area, suffered a sprained left shoulder, while placing 50-pound bags of product on a pallet.
