An unintentional roof fall was discovered on 10 North Mains, Primary Escapeway (Intake) #4 Entry, #4 Break, spad # 10693. Crew was pulled outby affected area, area is being dangered off, and the lifeline is being re-routed. MSHA and WVMHST Officials have been notified. No injuries or entrapments have occurred.

situation still under investigation so far information gathered is startled worker tripped and broke spine, further information still being gathered.

EE was stung by a bee

EE was descending the stairs to the office (4 steps) when EE's leg gave out and EE nearly fell. EE's right knee started showing

The Operator was notified on 8/24/2026, that the employee has been diagnosed with Occupational Hearing Loss.

The employee was attempting to remove a broken adjusting rod on a lower apron on a 1620 Metso crusher. They used the hydraulic rams to relieve the tension on the adjustment rods and was attempting to remove the broken nut/adjusting rod when the right hand hydraulic cylinder failed and allowed the adjusting plate to make contact with their right hand.

Employee was completing paperwork near EE's toolbox in Bay 4 of the Truck Shop when a gust of wind blew dust/debris into EE's right eye. The employee was wearing safety glasses at the time of the incident.

Field Loss fault on #3 Hoist

A miner was carrying a bundle of victaulic grips across the 5200 station when their knee buckled. The pain increased and mobility got worse as the day went on. The Dr diagnosed it as arthritis and put them on restricted duty for a week.

An unintentional roof fall was discovered on 9 North Mains in the Primary Escapeway (Intake), #3 and #4 Entries, from Brk #37 to Brk #38. The fall is approximately 20 ft. thick. All affected personnel were removed outby the fall, except those required to perform support work. No injuries or entrapments occurred. MSHA and WVMHST Officials were notified and are currently on-site.

The employee was performing normal job duties at the S Tank-manifold when they was bitten/stung by an unknown insect. Employee went to ER and was perscribed Benadryl and Prednisone.

Injured was cutting piece of mine belt off of an Irwin car in 5 block St Leo (8 Main Line Belt) and when cutting through the belt the knife slipped and caused a laceration to their right upper leg, anterior side, that required 5 sutures to close. No problems with equipment or compliance issues, training or PPE.

While changing an idler, the miner's right hand got stuck under it, causing a small laceration to their right index finger.

Employee was carrying hydraulic oil containers from shop to loader and sprained ankle while walking over concrete pad that had mounds of topsoil on it.

An employee sustained a cut to their right pointer finger while working on a reamer machine. While the employee was adjusting the threading portion of the threader, their hand slipped forward into the reamer portion of the machine, resulting in the injury. The employee was wearing Cut Level 4 resistant gloves at the time of the incident.

Group of Employees was sliding trougher. Trougher slid farther than expected and came off iron supports, and millwright apprentice got hand caught between trougher and support iron, causing contusion, bruising, and laceration to right hand.

Employee was working the backend of guillotine. The operator moved the leveling table and the slab they were cutting bounced. The employee had hand on table and the the slab smashed their right hand ring finger.

A customer truck driver came to pick up a load of bulk cement when EE experienced a personal health related issue.

Laceration of the right shin above the ankle. A large rock rolled off of C2 at U856 Sylvania striking employee on the right shin. The laceration caused by the rock required 16 sutures to remain closed.

Injury, fractured right fibular. Employee slipped on loose clinker material resulting in fracture

The Employee Received a 17.75% PPD for hearing loss.

During a maintenance task employees were tasked to replace 12' section of drip rail flashing material when a gust of wind displaced it impacting employees chin.

EE lost EE's footing, causing EE to lose control of the hose. Which then whipped around knocked EE down and hit EE in the face and head.

A layer in the primary hall road fell at the intersection of 0-F. No miners or equipment were injured.

At 8:35 AM, Employee radioed that they were having difficulty breathing. I met them at the ready line, where they reported chest pain and a migraine. They initially declined medical assistance but later requested it due to concerns about their heart and family history. I immediately called 911, notified Safety and my supervisor, and transported EE to the front gate to meet EMS.

Employee was putting material on the office bulletin board. As they stepped backward, they tripped on a box of paper and stuck out their left arm to brace their fall. The fall resulted in a fractured humerus.

While Place changing miner, employee stepped on a piece of coal twisting EE's ankle.

LEFT AND RIGHT CTS

While checking sanders on the motor in the Miracle Run Rock Dust Chute employee was walking around the motor and struck their left knee on the coupler, causing a contusion. No problems with equipment or compliance issues, training or PPE.

J-Mains #2 belt entry XC 61 had a small belt fire that need to be reported. The approximate time of the fire was 8:37 PM and was called approximately 45 minutes late.

An unplanned roof fall occurred on 8/20/2026 at approx. 12:40pm. The falls measurements are undetermined. The fall occurred on Mainline 14 Track at 14-02 crosscut, track to neutral. The area was posted on all sides, dangered off, and will not be cleaned up.

Injured employee was in the process of lifting a five (5) gallon bucket of bolts into a pick-up truck, as they lifted the bucket into the truck they felt a sharp pain in their back.

Rigid frame water truck drove onto right hand side berm, causing the truck to lay over. Employees are was caught between the cab of the truck and the ground

Injured was changing a bottom roller at 45 block on 9 North belt and while attempting to remove a roller retaining clip a piece of material entered EE's right area, resulting in a corneal abrasion. No problems with equipment or compliance issues, training or PPE.

Miner was pinched between a vehicle and the crushing plant electrical cabinet

Employee was using a punch and hammer to removed baffle bolts from the number 5 blunger and hit their left middle finger with the hammer causing a laceration.

Employee was walking by a belt winder in the supply yard when the employee tripped on a piece of curtain that was being used to cover the belt winder. Employee fell and landed on their left hand. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was aligning the top of the cyclones with a wrench when the wrench slipped out of position causing the cyclone top cover to cause a laceration to the R index finger.

When the St Leo escape hoist was being test ran (as part of MSHA's regular E01 inspection activity) it was discovered that it would not start. This was relayed to mine management's safety department at 1:43 pm 8/19/26. Through troubleshooting it was discovered that the starter fly wheel teeth had worn down to the point that the fly wheel would no longer engage.

Employee was climbing down from Front Shovel when footing slipped and they held on with right arm and there was a popping noise in right shoulder and there is a tennis ball size hard knot on their shoulder. Employee has an AC joint separation.

A mechanic working in the garage reported having some dirt in EE's eye. EE was not able to state a specific event on how the dirt got into EE's eye. After flushing the eye out, an appointment with an eye doctor was made so EE could be evaluated. It was determined that EE has a minor scratch on the eye and was prescribed antibiotic eye ointment.

A haul truck driver was traveling on the right side of the haul road and veered hard left rolling the truck onto its side. The EE stated the steering wheel pulled hard left.

The employee was on the #1 section in the # 10 entry installing the last bolt for the cut when a piece of rock measuring 20"X8"X1" fell striking the employee on the back of the left arm just above the elbow. Causing a 1.5-inch laceration. the entry was approximately 19.5 feet wide and 70 inches high. The employee received 6 sutures.

Employee was working on the 9W CM Section. Was walking from the face to the auxiliary fan when EE tripped on a rock and fell down, landing on EE's SCSR EE was wearing.

Employee was hanging cable around the P.C on 14 section, employee stepped in a rut beside the high voltage tub and rolled EE's left ankle. Fracture to left fibula

Subject was putting a guard in under a conveyor when EE heard a pop in EE's right elbow. EE went to the doctor the next day and was placed on light duty pending further medical review.

While using a sledge hammer to drive pins in loader, debris flew into face/eyes

EE was walking away from EE's stocking truck and rolled EE's left ankle

Employee was working on the 9W CM Section as a Roof Bolter. EE was pulling the trailing cable off the cable reel when EE felt pain in EE's lower back. Was diagnosed with a lower back strain and missed 4 days of work. Employee has training and experience and wearing required PPE.
