Employee was getting ready for lunch when they felt something in their right eye, the employee was to the bathroom to flush eye out but was not able to get the material out of their eye. the employee was taken to a clinic where the material come out on its own. The employee was prescribed eye drops.

The employee was cutting a zip tie with a utility knife in right hand, after cutting through, the forward motion of their arm/hand allowed the knife to cut into their arm about three inches above their wrist. Three sutures were used on the laceration.

Exposure to Coal Mine Dust

Employee smashed a finger between conveyor leg and conveyor.

While changing the seat air switch on the HE85 Loader. Had the seat braced in full up position. While unhooking the seat switch, the brace fell out and the seat fell approximately 8 inches down on to EE's left hand. This resulted in a primary injury to the pinky finger.

Employee was collecting trash from receptacles in employee parking lot and putting the trash in a pickup truck. They exited the truck to pick up trash and while employee was outside the vehicle, the truck began to roll backwards. They tried to jump into the truck to stop it and while doing so, tripped. The truck rolled over their right leg and fractured their right foot.

CONTUSION OF RIGHT FORN WALL OF THORAX, CONTUSION OR RIGHT SHOULDER, SPRAIN OF LIGAMENTS OF LUMBAR SPINE

I.E. was carrying an air tank to the face when EE slipped on shield #2 and the tank fell on EE's finger.

Subject was exiting a work pick up truck when EE twisted EE's left knee. Subject continued to work and went to doctor on 7/14/2026 and was given clearance to work but with restrictions.

EE felt 3 pops in EE's shoulder while on EE's forearms.

Employees were removing jacks from under #2 kiln tire (jacks free of any weight). Employee was guiding jack off the base; it came off awkwardly and pushed EE off balance. EE placed EE's right arm out to catch myself and felt a pop in EE's shoulder.

Employee was cutting bolt with utility knife when it slipped hurting EE's left upper leg.

A ROOF FALL ABOVE THE ANCHORAGE OCCURRED IN CROSSCUT 12 BETWEEN ROOM 78 AND 79 THE FALL MEASURE APPROXIMATELY 20 FEET LONG BY 17 FEET WIDE THE MINE WAS MINING TOWARD A RIVER VALLEY WHICH MAY HAVE INFLUENCED THE FALL

The employee was conducting scaling activities with a bar in a previously bolted row. As EE attempted to remove the first piece of loose rock an additional piece of rock fell striking the employee. As a result the employee received 3 staples to close a laceration to the head, a laceration to the left thumb receiving 2 sutures and a small fracture to the tip of the left thumb.

An unintentional roof fall was discovered on 9 North Mains, in the Intake, 2 Breaks outby spad # 10634. The fall is approximately 30ft. long, 10-12 ft. thick, and 18 ft. wide. No injuries or entrapments occurred as a result of this incident. MSHA and WVMHST Officials have been notified.

An employee was climbing the ladder to get in the cab of their loader and felt pain in their right thigh. They sought medical care and was written off work until 07/20/2026.

The employee applied force to the ignition key without maintaining proper grip and control. Contributing factors: use of a key ring with multiple keys; failure to follow supervisory instructions; lack of hazard recognition. Root cause: noncompliance with operating instructions and use of an inadequate key system for safe equipment operation.

Exposure to Coal Mine Dust

The EE was performing services on the AC condenser of a 770 Caterpillar haul truck. The EE right hand slipped during repair and came into contact with the condenser fan causing a laceration to their right pinky finger that required stitches.

Injured was walking and shoveling spillage along 6 belt at 120 block when a rock dislodged from the top and hit them in the back near the left shoulder blade area, resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

Employee was dropping bolt machine cable. The employee unhooked one side of the nylon hanger to let the cable drop. The weight of the cable spun the hanger around. The metal hook (on the hanger) hooked into the frame of safety glasses and the hook slid up the face cutting the bottom eyelid. It took 6-8 stitches to close the wound.

Employee was walking up to the continuous miner, as they approached the miner they slipped and fell striking the right side of their face on the miner resulting in fracture to cheek bone.

Employee was lifting a timber, dropped it, causing it to smash EE's right thumb between a crib and the timber. This resulted in a laceration requiring three sutures.

While changing the trip rope on 7450 dragline, employee was feeding the end of the cable back into the block when EE lost grip causing it to swing back at EE and strike EE just above EE's left eye at EE's eyebrow. EE received a laceration and required 3 stitches.

Employee was not wearing PPE when they were opening a tin of welding rods and cut their fingers

employee alleges the auto darkening on welding hood did not engage and their eyes sustained minor flash burn

Employee was using a utility knife to cut conveyor belting to cover the scale access point when the knife slipped and cut EE's left thigh. Employee received three stitches.

On 7/8/26 at approximately 8AM EE was injured from a screen on the crusher, resulting in 3 puncture wounds to EE's leg. EE refused to go to a doctor at that time. On 7/13/26 EE decided to go to a clinic. EE had an infection and was given a tetanus shot and oral antibiotics. The doctor didn't give EE any time off and EE returned to work the same day.

EMPLOYEE WAS CUTTING A PIECE OF METAL OFF OF A BASKET ABOVE EE; THE WELD BROKE ON THE OPPOSITE SIDE OF THE BASKET AND A PIECE OF METAL FELL ON EMPLOYEES RIGHT FOOT.

Flushing out lines on the 9 dump, while attempting to adjust the wobbler the wobbler tip broke off and raffinate solution sprayed the employees head/face area. The employee used their hands to wipe their eyes and got solution in their eyes.

Employee suffered injuries to the top of EE's left foot and toes when a piece of stone fell from the rib and penetrated EE's leather mining boot just behind the steel toe.

There was a unplanned roof fall at #11 Main North C, XC 50, #10 Entry.

During a pre-trip inspection, the rigger was directed to the opposite side of the engine compartment. Instead of climbing down to go around, they walked across stored wood mats on the crane. The mats shifted, causing them to fall and fracture their ankle

Employee sustained laceration on forearm from bolt on hand rail.

Subject completed his shift and was getting into their vehicle when they were stung by a wasp. Subject is allergic so they administered an EpiPen, drove home, and had their spouse take them to the ER. While at the ER, employee was given Benedryl. No further actions were taken.

Coworker was on uneven surface, mis stepped, lost their balance and fell. During the fall, EE hurt EE's wrist.

The IE was operating a stand-up skid loader on a concrete pad removing spillage. Employee was dumping a bucket over the edge of the pad, machine traveled too far over the edge of pad. Machine tilted forward causing the right ankle to become wedged in a gap on the machine.

Employee was having continuous left shoulder pain from performing manual labor. Resulted in left shoulder sprain.

Employee was drilling a hole into poly to line up the hole in the metal sheet and employee drill hit the metal hole causing the drill bit to get caught and spun around hitting the employees' nose and face.

EE was helping collar a drill hole for the miner when a rock fell from the face and struck EE's arm. The rock cut EE's right forearm. The miner cleaned EE's arm with the first aid kit and bandaged it. Then brought EE to the superintendent so we could assist in getting EE to surface.

During cone crusher reassembly, a two-person crew installed lock pins/bushings - one swinging a sledgehammer, the other guiding the ram and signaling the crane operator. The guide called 'hold on' without confirming it was heard, then reached into the strike zone. The hammer struck EE's hand, displacing and lacerating EE's left middle finger.

Employee was shoveling material and felt a painful pop in EE's lower back.

Injured stopped trip to see if trucks were on track near 28 block 9 North and while standing next to de-railed block car a cube of block slid off and landed on their left foot, causing a contusion. No problems with equipment or compliance issues, training or PPE.

Employee was helping to install a transmission in a Mack Coal Truck when a piece of dirt fell into EE's right eye. (Employee was wearing safety glasses at the time of the accident)

Team member climbed onto a water truck to check the water level. As they were climbing back down, their left ring became caught on a step, causing a snag incident.

As employee was climbing into their truck at the mine, their hand slipped off the handhold and they fell to the ground. Employee sustained a strain to their right hip and back.

Employee was using folding utility knife to remove broken plastic fitting from tube. Knife folded and cut employee's right index finger. Employee received 3 stitches
