Miner was walking close to a front end loader when they stepped on something (possibly a rock) and injured their foot/ankle. their ability to walk was impaired.

We were replacing the splices on electrical connections. Employee was cutting through one of the resin splices when the knife skipped and imbedded in hand between the thumb and pointer finger. They cleaned it up and did not want to go to the Dr., but It was deep enough that they at least needed to get it checked out. It took three small stiches.

Troubling shooting knocking sound on Genset, a catastrophic failure happened causing hot oil and metal fragments to spray out contacting the injured employee

The IE was removing a 10-inch valve in the RO Plant and had removed several bolts. When the IE removed the 2nd to last bolt, the valve swiveled and pinched the IE's ring finger between the collar of the valve and the pipe

While pushing themself off the ground they felt discomfort in left hand.

The employee was descending a haul truck and got their boot stuck on the bottom rung. This caused their weight to shift and employee was unable to hold themselves and fell backwards (16 inches) onto the ground

EMPLOYEE WAS CUTTING T-3 CHANNEL WHEN A SMALL PIECE OF METAL SHAVINGS BROKE LOOSE AND ENTERED UNDERNEATH HIS SAFETY GLASSES, STRIKING THE EMPLOYEE IN THE LEFT EYE.

Employee smashed the tip of their right middle finger while removing a blower housing, no fractures were reported.

The employee was working on a metal skirting in building #2, which at some point was attached to the crane, as the crane was being used to lift/move the skirting it sprang loose and struck the employee on the knee causing them to lose their balance and hit the middle of their back against the chute.

#1 Hoist went down, Weather came through the area and there was a power glitch

Man basket popped up with employee inside and they struck their chin on the railing.

Injured was assisting with removing a 4 wheel hand cart off the Metz elevator that was loaded with roof bolts when the front wheels of the cart broke off driving the handle of the cart into their left lower leg, resulting in a fractured fibula. No problems with equipment or compliance issues, training or PPE.

Employee was looking upward at the drill and felt something fall into their right eye while wearing safety glasses and then felt a pop followed by tingling.

Operator was running the BC9 south dump when they felt a pain in their lower back at approximately 8pm on 08/27/26 and continued to feel stiffness and pain throughout the rest of shift. Reported injury before shift start the next day.

Miner was loading boxes into truck lost footing and fell against the truck, striking left shoulder on truck.

Employee was attempting to replace the conveyor belt and cut a finger on the drive belt sheave. No stitches were received, only work restrictions.

Employee stepped off of a ladder rolled an ankle.

A mechanic's left index finger became pinched between sling and regrind ball mill when performing routine maintenance. Event went reportable on 08/25/2026.

While a worker was standing near a band saw roller stand. A 1-1/2 sch 40 pipe was resting on rollers. Another worker accidentally bumped the pipe. The pipe rolled off the stand and struck the worker's right foot, causing a contusion.

Clinker Silo elevator become disabled approximately 130' with a contractor scaffolding employee in the elevator. The call to supervisor and safety was made. Employee was not injured and was doing ok communication by radio and cell phone. Employee was safely taken to the ground with a 150" manlift with no injuries or further incident.

A roof fall has occurred on the #1 unit of the 1I panel in #4 entry. From spad 20+20 to 20+85. The fall measured 25ft x 6ft x 19ft. The fall will not be loaded out and the area has had additional support added.

An unintentional roof fall was discovered on 10 North Mains, Primary Escapeway (Intake) #4 Entry, #4 Break, spad # 10693. Crew was pulled outby affected area, area is being dangered off, and the lifeline is being re-routed. MSHA and WVMHST Officials have been notified. No injuries or entrapments have occurred.

Employee was climbing down the ladder/steps of Loader #10 and the bottom step broke causing them to go to the ground hurting their right knee.

Employee was pushing buckets and felt a pop in lower right back.

Miner was reaching under a suspended load when it fell on their fingers.

Employee stated they were holding a pin hammer when another employee struck the pin a piece of metal flew off striking the employee in the arm causing a laceration. Employee sought medical attention and was given 3 sutures. This incident is being reported as a medical treatment

EE spotting haul truck under loader, the loader operator dropped full bucket on left bedrail, jostling the EE in the cab of the haul truck. Seen at clinic, xrays negative for fracture, released to modifed duty, worked one day modified, seen on 8/26, released to full duty.

situation still under investigation so far information gathered is startled worker tripped and broke spine, further information still being gathered.

Employee was operating a loader when bit by spider in right ankle

EE was stung by a bee

EE was descending the stairs to the office (4 steps) when EE's leg gave out and EE nearly fell. EE's right knee started showing

The Operator was notified on 8/24/2026, that the employee has been diagnosed with Occupational Hearing Loss.

The employee was attempting to remove a broken adjusting rod on a lower apron on a 1620 Metso crusher. They used the hydraulic rams to relieve the tension on the adjustment rods and was attempting to remove the broken nut/adjusting rod when the right hand hydraulic cylinder failed and allowed the adjusting plate to make contact with their right hand.

Employee was helping to hang high voltage cable along 9 South Track, when a small rock came off the rib and hit the left side of nose. Employee was diagnosed with a nasal fracture.

Employee was assisting with a Vac-Truck hose in deep, muddy tundra conditions when unstable footing restricted movement and body positioning. While applying force to control the hose, their grip slipped and the hose contacted their right shoulder. Injury resulted in right anterior shoulder dislocation, restricted work duty.

Unplanned roof fall discovered reportable on 1 North Haulage at 23 block at approximately 8:45 pm on 08/23/2026. Fall primarily occurred over top of arches and steel sets that were previously installed through the area. Fall was approximately 60' long, 16' wide, and 25' high. Area was mined near June of 1989. No problems with equipment or compliance issues, training or PPE.

Employee was completing paperwork near EE's toolbox in Bay 4 of the Truck Shop when a gust of wind blew dust/debris into EE's right eye. The employee was wearing safety glasses at the time of the incident.

Field Loss fault on #3 Hoist

The haul truck operator was exiting and stepped onto the truck's walkway when their right knee hit a metal panel. This contact caused a cut on the right knee. The event went reportable on 08/22/2026.

Electrical control superintendent was troubleshooting inside the #10-MC-03 energized 4160-volt electrical enclosure when they contacted the phase to phase with a fluke T6-600 voltages tester. This cause an arc flash causing burns to their hands, arms, and face due to not donning any of the required PPE and the 4160 volt power circuit not being deenergized before the work began.

A miner was carrying a bundle of victaulic grips across the 5200 station when their knee buckled. The pain increased and mobility got worse as the day went on. The Dr diagnosed it as arthritis and put them on restricted duty for a week.

An unintentional roof fall was discovered on 9 North Mains in the Primary Escapeway (Intake), #3 and #4 Entries, from Brk #37 to Brk #38. The fall is approximately 20 ft. thick. All affected personnel were removed outby the fall, except those required to perform support work. No injuries or entrapments occurred. MSHA and WVMHST Officials were notified and are currently on-site.

The employee was performing normal job duties at the S Tank-manifold when they was bitten/stung by an unknown insect. Employee went to ER and was perscribed Benadryl and Prednisone.

Injured was cutting piece of mine belt off of an Irwin car in 5 block St Leo (8 Main Line Belt) and when cutting through the belt the knife slipped and caused a laceration to their right upper leg, anterior side, that required 5 sutures to close. No problems with equipment or compliance issues, training or PPE.

While changing an idler, the miner's right hand got stuck under it, causing a small laceration to their right index finger.

While performing a belt move in St Leo Mains #3 entry 163-165 block the injured was carrying belt structure and felt pain in groin and lower stomach area. Was later diagnosed with a strained groin. No problem with equipment or compliance issues, training or PPE.

The mine received a Self-Insured Employer Decision on 08/21/2026 from a medical report dated 4/30/26. The report identifies a possible 10% permanent partial disability from occupational pneumoconiosis. Employee has been retired since 9/15/18

Employee had completed their workday and was assisting another coworker with a flat that they had on their personal vehicle. While placing another jack in place, the first jack collapsed and the vehicle fell. Employee was hit by the falling vehicles bumper hitch. The employee was not on company time and was not using company equipment. Reporting due to it occurred on mine property.

Miner was locking down a bearing using a sledgehammer and missed the slugger wrench's mark. Hammer struck another miner holding the slugger wrench on their right wrist. Miner that was struck received a contusion.

Employee was carrying hydraulic oil containers from shop to loader and sprained ankle while walking over concrete pad that had mounds of topsoil on it.
