Employee was standing on the left side catwalk of a 777F rock truck, near the rear of the cab, when the dump bed came down, crushing him between the bed and the catwalk handrail.

Struck by flying object

The victim (mechanic) removed fuel injector from engine and then instructed his helper to turn the engine over. Diesel fuel and/or air came out of the 1/4 inch injector hole and struck him in the left eye and entered his brain, causing fatal injury.
