Employee was getting ready for lunch when they felt something in their right eye, the employee was to the bathroom to flush eye out but was not able to get the material out of their eye. the employee was taken to a clinic where the material come out on its own. The employee was prescribed eye drops.

The employee was cutting a zip tie with a utility knife in right hand, after cutting through, the forward motion of their arm/hand allowed the knife to cut into their arm about three inches above their wrist. Three sutures were used on the laceration.

While changing the seat air switch on the HE85 Loader. Had the seat braced in full up position. While unhooking the seat switch, the brace fell out and the seat fell approximately 8 inches down on to EE's left hand. This resulted in a primary injury to the pinky finger.

Employee was collecting trash from receptacles in employee parking lot and putting the trash in a pickup truck. They exited the truck to pick up trash and while employee was outside the vehicle, the truck began to roll backwards. They tried to jump into the truck to stop it and while doing so, tripped. The truck rolled over their right leg and fractured their right foot.

The employee was conducting scaling activities with a bar in a previously bolted row. As EE attempted to remove the first piece of loose rock an additional piece of rock fell striking the employee. As a result the employee received 3 staples to close a laceration to the head, a laceration to the left thumb receiving 2 sutures and a small fracture to the tip of the left thumb.

An unintentional roof fall was discovered on 9 North Mains, in the Intake, 2 Breaks outby spad # 10634. The fall is approximately 30ft. long, 10-12 ft. thick, and 18 ft. wide. No injuries or entrapments occurred as a result of this incident. MSHA and WVMHST Officials have been notified.

The EE was performing services on the AC condenser of a 770 Caterpillar haul truck. The EE right hand slipped during repair and came into contact with the condenser fan causing a laceration to their right pinky finger that required stitches.

Employee was dropping bolt machine cable. The employee unhooked one side of the nylon hanger to let the cable drop. The weight of the cable spun the hanger around. The metal hook (on the hanger) hooked into the frame of safety glasses and the hook slid up the face cutting the bottom eyelid. It took 6-8 stitches to close the wound.

On 7/8/26 at approximately 8AM EE was injured from a screen on the crusher, resulting in 3 puncture wounds to EE's leg. EE refused to go to a doctor at that time. On 7/13/26 EE decided to go to a clinic. EE had an infection and was given a tetanus shot and oral antibiotics. The doctor didn't give EE any time off and EE returned to work the same day.

EE was helping collar a drill hole for the miner when a rock fell from the face and struck EE's arm. The rock cut EE's right forearm. The miner cleaned EE's arm with the first aid kit and bandaged it. Then brought EE to the superintendent so we could assist in getting EE to surface.

During cone crusher reassembly, a two-person crew installed lock pins/bushings - one swinging a sledgehammer, the other guiding the ram and signaling the crane operator. The guide called 'hold on' without confirming it was heard, then reached into the strike zone. The hammer struck EE's hand, displacing and lacerating EE's left middle finger.
