INJURED WAS PUTTING OIL IN THE CENTER GEAR CASE OF THE #11 JOY MINER ON #2 UNIT WHEN A PIECE OF ROCK FELL FROM THE ROOF STRIKING INJURED IN THE BACK OF THE HEAD AND SHOULDERS PINNING HIS HEAD ON THE MINER. INJURED RECEIVED SEVERE HEAD INJURIES. INJURED WAS LIFE FLIGHTED FROM THE MINE TO ST MARY'S HOSPITAL IN EVANSVILLE, IND. INJURED DIED ABOUT 6:15 PM ON MAY 23, 2001.
