Roof fall a entry 7 so. n.w. at 55 18 & 05 approx. 20"" lg, 16 ft wide 6 ft high. this area had previous roof sloughing off. then was dud is not now causing any effect on went. found to be fallen about the achorage gone on 4-6-04. 6' resin grouted rods, grey shale, rared stone. now alone anchorage gone

FALL OF ROOF. D ENTRY, 7 SOUTH, MAIN WEST @ 5.5 15+30 (TRACK ENTRY) 003-0 SECTION, 400 FT OUTBY LOADING POINT. APPROX 30 FEET LONG, APPROX 20 FEET WIDE. APPROX 7 FEET HIGH. GREY SHALE WITH SOME SANDSTONE, COAL. FELL ON MANTRIP CAR.

Was lifting a section of track rail to place in scoop car with helper and hurt left rib area.

EE was assisting in putting cat drive in feeder, the feeders front lift jack fell and hit his left thumb. compliance met a factor.

EE WAS ATTEMPTING TO CUT OUTER JACKED OF CABLE. THE CABLE WAS STIFF DUE TO BEING COLD THE KNIFE SLIPPED CUTTING HIS RIGHT THIGHT.
