A caterpillar 777 haul truck rolled over an embankment coming to a complete stop and landing on all four wheels.

Over-exertion in lifting objects

Lifting garbage bags into trash dumpster using both hands. Heard ""pop"" in right shoulder and felt pain.

Stepped out of vehicle onto icy running board. Twisted ankle, not wearing PPE ice cleats that company issued to him at that time.

Slipped on ice going into truck shop building.

EE was cutting a piece of pipe with a reciprocating saw. One beyond the cut, the saw blade hit metal grating and kicked back. The employee sustained a laceration to his left hand between the fore and middle fingers. 9 sutures were required to close the wound.
