During concrete invert pour, the concrete car was being pulled to surface, the screed cable supposedly got on the concrete car, which pulled the screed up hill, causing screed to come into contact with employee

Employee was stung in throat/neck area by a bee. EE is allergic to bee stings and was taken to Broaddus ER by co-worker. NOTE: EMPLOYEE WAS RELEASED TO COME BACK TO WORK (FULL RELEASE) ON 09/15/2021 WHICH WAS EE'S NEXT SCHEDULED DAY DUE TO EE'S WORK SCHEDULE ROTATION. EE WAS QUARANTINED ON 09/15/2021 DUE TO COVID-19 PROTOCOLS.

Employee was stung by bee and they had reaction to sting.

While employee was pushing on concrete form to get in position, EE's hand slipped off and got caught between form and bolt smashing finger.
