Elevated level of methane was detected by the mine monitoring system in the bleeder area for the longwall. Power was removed from the mine and the mine was evacuated.

A coal outburst occurred 38 feet outby the longwall face in the # 2 entry, the outburst dislodged 8 rock pops that were set on the rib of the walkway. The affected area was 23 feet.

Employee was puting a pump through a man door and herniated a disk in his lower back.

Two employees where lifting a guarding in place for a tail piece. The injured employee let go with his left hand to chain the guarding into place and pulled his muscles in his right arm.

Two employees were packing a top roller cluster when one employee dropped the cluster & it hit the other employee in the lower back.
