Accident type, without injuries

Fall was discovered in the No. 6 intake entry of No. 1 North at survey station No. 563. The fall measured approximately 20' long x 20' wide x 6' high and will not be cleaned up. Ventilation was not affected. Passage of men was not affected. There are no injuries or equipment involved.

Struck against a moving object

EE was tramming a battery powered scoop & ran over a rock which bounced him in the deck of the scoop & bruised his back.

Over-exertion in lifting objects

The employee was helping install belt conveyor structure after a belt move. He was helping lift belt structure in place when he strained his back.

Struck against stationary object

The injured was riding a rubber tired mantrip. As the mantrip traveled through a set of doors, one side of the door began to close. The injured raised up to close the door and struck the framework of the door. This resulted in a laceration to his forehead which required one (1) suture.
