Fall down stairs

EE was leaving his work location in the Shipping Tower, decending the steps from the 3rd level. Prior to reaching the 2nd level landing, he looked back up at the 3rd level and noticed he had left the Load Out Chut in the down position. He was not paying attention to the steps and and missed the bottom step, twisting his right ankle, examined by MD, put on restricitons.

WHILE INSTALLING SHOTCRETE IN ROTARY KILN, EMPLOYEE WAS STRAINING TO GET A CLAMP ON PUMPING HOSE. FELT PULL IN CHEST. CONTINUED TO WORK FOR A BIT THEN LEFT JOBSITE. COULD NOT BREATHE, HAD CHEST PAIN. EMPLOYEE WAS TAKEN FOR MEDICAL ATTENTION.
