CLEANING THE PLANT AREA OF THE QUARRY WENT TO PICK UP A PIECE OF SCREEN AND SLIPPED WITH HIS OWN FOOT, FELL AND HURT HIS LEFT WRIST.

HE WAS WORKING WITH A DUMP TRAILER AND A PIECE OF METAL FELL FROM THE DUMP, HITTING HIS LEFT SHOULDER. PUTTING DUMP TRAILER ON THE HITCH.

Struck by concussion

CLEANING A HOLE, NEXT TO HIM WAS A HOSE WITH OILFOR THE DRILLING MACHINE (TAMROCK RANGER 700). THE HOSE SPLASHED SOME OIL INTO HIS EYE.
