Fall from machine

Upon information provided injury occurred while tarping load. EE fell from trailer to the ground.

Struck against a moving object

Employee parked Haul truck and complained about Neck/Back strain. Employee was sent to company doctor.

Struck by flying object

Employee was torching a part on the cone crusher when a piece of debris got into EE's eye. Employee was following company policy and wearing safety glasses. Employee reported the injury the following day 7/11/2020 and was sent for medical review on 7/13/2020 to evaluate.

Over-exertion in lifting objects

Employee was setting concrete blocks on stem wall. Reported that shoulder was sore. Rested it for a couple days with minimal exertion and was then sent to the company doctor.
