Miner was washing out plugged feed tube from sand tank to sand screw from discharge end with water hose. When material broke free, hose whipped out of pipe and struck miner in face resulting in five stitches to left eye.

Miner was taking a light off light pole, unhooked the brace the light swung around and hit miner in right ear and head.

Miner was cutting a poly grease line with a knife. The knife slipped and the miner cut his left thumb, resulting in 4 stitches.

Struck by flying object

Miner was performing maintenance (welding) inside of feed hopper. Miner inside hopper needed a bolt to fill a hole; asked miner on outside of hopper for bolt. Miner on outside of hopper tossed bolt to miner inside hopper; miner inside hopper missed bolt and chipped front tooth in process. Accident is reportable due to follow-up visit at orthodontist office.
