One spring on one hoist brake of the 8 Shaft hoist broke. No one was stuck on the cage and 2 escapeways were maintained throughout the changing of the spring.

On the 8 Shaft Hoist an electrical contactor failed, the hoist would not operate. No one was stuck on the cage or injured, the contactor was changed and the hoist returned to operation.
Employee was lifting a railcar lid. The lid jammed half way up and the employee strained his back. Company was notified of work restrictions on 10/31/11.
While the employee was traveling to the mining section to begin work the nut retaining the pitman arm fell off causing the jeep to veer to the left climbing the rib and rolling over. Initial examination at the hospital showed no injury. Reexamination on 7/8/11 showed a fractured vertebrae. At that time the doctor put him off work.
Employee was using a 1/2 inch drill to drill a hole in sheet metal. The hole saw grabbed, causing the drill to spin out of his hand. The drill had a side handle attached and the twisting of the drill caused the side handle to strain his left thumb. Injury became reportable when the employee had surgical intervention on 7/19/2011.
Checking tail of CB-2216; stood up & hit head on a beam near the tail pulley(approx. 5 1/2' high). The impact caused dizziness and head and neck pain.
Slipped on ice that was on the gravel on the south side of the fire truck garage while walking to the pickup.
Employee was aligning the load spout to the Railcar. He was aligning the spout with his right leg. He was supporting his posture and gripping the hand rail. As he pushed with his right leg. He felt a sharp pain run through the back side of his right upper leg which resulted in a torn hamstring muscle.
Employee was manually handling filter leaves and felt a pop and then pain in right arm. Employee diagnosed with detached bicep tendon.
Lifting a piece of pipe to install with a come-a-long; pipe hung up - felt a pop in left shoulder & pain soon after.
A miner was barring down some top that was over a disconnect box. When the rock hit the box it bounced and struck the miner's right foot, fracturing one bone in the right foot. The miner had X-rays taken on 12/19/2011 and that is when the broken bone was discovered. Prior to the 19th the injury was not thought to be serious.
Stepped out of truck, reached inside to get chocks, slipped on ice injuring his back.
Employee was moving a 6"" vacuum hose and felt pain in his shoulder. Employee was prescribed Physical Therapy.
Worker on his own decided to assist moving the material being lifted away from the wall by pushing the lift cable instead of the pallet of material. Had his gloves come into contact with top of the tugger pully pinching his left hand (little and ring finger) worker was wearing heavy leather gloves.
Fell from ladder while attempting to check a nuclear source. Had medical related episode that may have resulted in incident/injury. Ended up staying overnight in hospital & had sutures in head. Unsure at this time whether it will become a recordable incident/injury.
Employee reported pain in L Wrist which was diagnosed as inter carpal instability. Accepted as work related on 5/17/11 when medical documentation was received & reviewed that stated diagnosis.
