Fall from machine
employee was getting out of haul truck and he missed a step and fell to the ground. He injured his back.
Struck against stationary object
Employee was greasing equipment at beginning of shift. He threw the grease hose over the bucket of the front end loader. He caught his left fore arm on the edge of the bucket during the motion of throwing the hose. He received a laceration of the left fore arm upon contact.
