Struck by falling object

STONE DEBRIS WAS STRUCK IN REMOVAL TRUCK FALLING AND HITTING MINER IN SHOULDER AND LANDED ON HIS LEFT FOOT.

Fall from ladders

Climbing up a ladder to a height of between 4-5'. When on the last step before stepping off of the ladder it folded and the EE fell approx. 4' to the ground injuring his back and twisted his ankle.
