Accident type, without injuries

Unplanned roof fall at #61xc, #2 entry, ft. #9068 on #4L travel road. Fall occurred in intersection supported with 84"" tr bolts and 12'cable bolts. Fall dimensions 19' w x 19' L x 8'5"" h. Fall did not cause injury, impair ventilation, or entrap miners. Fall area developed 7-1-2015.

Struck by falling object

Was putting screw jacks in to hold a pony boom up, supports dislodged and the pony boom came down hitting shoulders knocking EE to the ground. *On October 1, 2015, the injured miner died. On October 13, 2016, MSHA's Fatality Review Committee determined the death to be mining related and chargeable to the mining industry.

Fall from ladders

He was using a stepladder while sealing a stopping. He reached over to get more plaster out of the bucket, the ladder tipped over; he then leaped off and landed on his feet. Felt pain in his mid-back.

Struck by falling object

Employee was working on the right side of the cutter head of CM. While replacing water lines, that cover on miner that protects the water line fell on right hand causing an injury

Fall to the walkway or working surface

Coming out of a Kennedy, went to step down off bank and slipped on loose rock, threw off his balance and landed on his shoulder.

Struck against stationary object

He was hanging tubing and went to push tube in tight. A piece of fiberglass got into his index finger on his right hand. He was seen on 8/26/2015 by an Orthopedic doctor and was taken off work.

Over-exertion (Not Elsewhere Classified)

Employee was working on a shield hauler, while walking around the left side of the machine, he stepped on a rib rash and rolled his left foot inwards and heard a pop.

A Pillar Innovations employee was working in the 5 Left Mains. The employee must mount and dismount the belt line which is elevated approximately 3' 1/2"" from the ground. The employee stepped off the belt line and on to an uneven surface, rolling his ankle. Employee suffered an ankle sprain, the doctor did assign an air cast to be worn out of the medical office.

Caught in, under or between a moving and a stationary object

Employee and another crew member was pulling out top rollers with impact rollers out of the Main North belt. One end of the 72"" roller slipped off the belt rail while they were holding the other end causing EE's left ring finger to get pinched between the roller frame and the wall.

He was moving power cables. The cat head came down and struck him in the mouth breaking his tooth.

Struck against stationary object

Prying on wear ring on pump. Screwdriver slipped, He hit right thumb on face of pump. (received stitches)

Struck by falling object

He was having trouble with his drill steel and stepped down, his bolting partner stepped up on his platform and was trying to get the drill steel out of the hole. He shook the supply to try knocking some plates down, one struck the injured on the hand.
