HAD ROCK FALL AT #37 ON 1ST NORTH TRACK. 25' X 20' X 6'
ROCKFALL IN #6 RETURN AT 176 + 90 - #1 ENTRY. 8' X 20' X 210'.
ROCKFALL IN 3SME RETURN AT 13+30. 20' X 40' X 5'.
EMPLOYEE WAS PULLING ON REBAR MAT TO PUT CRIBBING UNDER IT WHEN HE PULLED A MUSCLE IN HIS BACK.
EE WAS OPERATING CRANE, WHEN HE SAID HE HAD FLASH BURNS TO HIS EYES.
EE WAS WALKING DOWN SLOPE TO CLEAN OUT AROUND 13 HP FLYGHT PUMP ON WEST SIDE OF SLOPE, WHEN HE STEPPED ON WET GUNITE CAUSING HIM TO SLIP AND FALL ON HIS LEFT ANKLE.
EE WAS GOING IN SLOPE AT START OF SHIFT, HE HAD MUD ON HIS FEET WHEN HE SLIPPED AND FELL. ACCIDENT WAS NOT REPORTED UNTIL 10-23-01.
EMPLOYEE WAS HELPING ANOTHER EMPLOYEE LIFT A 6 FT SUPER STUD WHEN HE SAID HE HAD PULLED A MUSCLE IN HIS BACK.
EMPLOYEE WAS ADJUSTING FORKS ON LOADER WHEN HE SAID HIS BACK STARTED HURTING.
EMPLOYEE STATED: HE WAS PINNING HIGH TOP AND A ROCK BROKE LOOSE STRIKING HIM ON THE RIGHT SHOULDER AND ARM. NOTE: EMPLOYEE DID NOT START MISSING WORK UNTIL 10/31/01!
EMPLOYEE WAS MOVING A BEARING AND CAUGHT HIS FINGER BETWEEN TOW BEARINGS.
EMPLOYEE WAS PUTTING IN ROOF BOLTS WHEN A ROCK WAS VIBRATED LOOSE. IT FELL HITTING HIM ON THE BRIDGE OF NOSE.
EMPLOYEE STATED: HE WAS MOVING EAGLE SEAL IN A BUCKET AND FELT PAIN IN LOWER BACK.
EE WAS PINNING HIGH TOP PIN CAME OUT OF WRENCH AND HIT HIM IN THE HEAD.
EE WAS WELDING REBAR WHEN HIS HOOD KEPT COMING OPEN CAUSING HIM TO BURN HIS EYES.
EMPLOYEE SAID HE WAS WALKING UP SLOPE WHEN HE STARTED HAVING CHEST PAINS. HE DID NOT REPORT THIS TO ANYONE.
