Accident type, without injuries

Rock measuring 5 1/2' X 10' X 18' wide fell ont CM while cutting coal. No vent. or travel of people or equip were affected. Both MSHA & OMSL were notified, and investigated.

Accident type, without injuries

About 4 A.M. discovered fall entry #6 on 001 section, in return, 60' inby spad 2184. Fall was 55' X 20' X 5', there was no equipment involved, ventilation was not affected, passage of people was not affected. Area was breakered off, and we have no plans to rehab this area. Both MSHA and KY OMSL was notified.

Fall onto or against objects

EE walking through intersection of #3 entry, trip over water line, and fell onto SCSR injured low back area. Taken to ER, x-ray negative, released to RTW 9/2/09. On 9/2/09 saw Dr. at Family Health Center, taken off work..

Fall from ladders

Employee was inspecting bearing on crusher. When getting down from crusher, the ladder fell approx 4 feet, landing against plant wall. Employee saw doctor, but continued to work. M.D. referred him to Neuro. Last day worked 12-30-09.

Over-exertion in lifting objects

Employee had walked the main belt-line and came outside. He was at the drift mouth at A-1 belt. Employee bent over to pick up a piece of chain link wt. about 20 lb. He developed a sharp pain in his low back. Dr. at the E.R. examined him and said he had acute muscle strain, told follow up with family Dr., he saw family Dr. 8-5-09, and was taken off work. (Mine Closed 2-10-10)

Fall from machine

Employee was exiting Cat. 990-G end loader. When he stepped onto the ground with his right foot, his left foot got hung on the step causing him to fall to the ground; resulting in injury to the left knee.

Struck by... (Not Elsewhere Classified)

Employee was filling in for absent RB operator & had bent bolt to install, and was straightening it. Bolt hd. & chuck not aligned. Employee bumped rotation to align bolt & chuck; caused bolt to spin - plate struck right forearm caused injury. Employee taken to E.R. for treatment.

Struck by... (Not Elsewhere Classified)

Employee bent bolt to install, feeding bolt into roof, bolt kick out of drill pot striking left wrist. Continued to work until 4-13-09. Went to Dr. then taken off work.

Struck by falling object

Employee was watering scoop batteries, the lid fell and struck employee in mouth, cutting lip and chipped front teeth.
