Accident type, without injuries

A roof fall occurred in the #5 entry of the 6th Southeast Bleeder Panel approximately 10' inby survey station 5493. The fall is approximately 140' long x 19' wide x 6' high. The fall was found by the mine foreman and MSHA inspector while traveling the bleeder entries. The fall was reported to MSHA hot line on 2/29/2012 (date fall was found).

Struck by falling object

EE WAS SETTING A TIMBER ALONG THE RIB WHEN THE RIB ROLLED OFF AND STRUCK HIM, TRAPPING HIM UNDERNEATH THE RIB, RESULTING IN FATAL INJURIES. THE TOTAL RIB ROLL WAS APPROXIMATELY 26'LONG x 8' HIGH x 18"" THICK. ACCIDENT WAS REPORTED TO MSHA HOT LINE ON THE DAY OF THE ACCIDENT.

Fall from scaffolds, walkways, platforms

Worker took it on his self to cut belt line down without the use of the equipment that we was using to take it down with. He cut it then started walking away from it as it was falling. Then it caught the catwalk he was on and flipped him over the double hand rail on the catwalk and fell to the ground about 20 ft down.

Over-exertion in pulling or pushing objects

EE was pulling on a canopy extention on a roof bolter and felt a sharp pain in his groin area. He continued to work his regular duties, but the pain persisted. He sought medical attention with his family doctor on 08/23/2012, at which time he was taken off work for a hernia.

EE was folding conveyor belt into laps. He was pulling the belt with channel locks. The channel locks slipped off the belt resulting in EE straining a muscle in his right side.

Struck against stationary object

EE WAS GETTING OFF A MECHANICS RIDE AND STEPPED ON A PIECE OF BELT AND TWISTED HIS LEFT KNEE.

Over-exertion in lifting objects

EE was building a brattice. He was on his knees, and as he picked up a block to place on the 5th row of the brattice, he felt a sharp pain in his back.

Struck by falling object

EE was raising the drill boom of the roof bolter to check the size of a busted hydraulic hose. The hose got fouled on the frame of the roof bolter, and when he raised the drill boom and looked under it, the hose came loose and struck him above the left eye, resulting in a cut.

EE WAS REPLACING A 12 INCH PIPE ON A DESLIME HEADER BOX. HE WAS HOLDING ONTO THE PIPE AND THE PIPE SLIPPED AND CAUGHT HIS FOREARM BETWEEN THE TWO PIPES.

Struck by falling object

EE HAD DRILLED A HOLE IN THE MINE ROOF AND THE DRILL STEEL STUCK IN THE ROOF. WHEN HE LOWERED HIS DRILL POT, THE DRILL STEEL FELL FROM THE HOLE AND STRUCK HIM ON THE LEFT HAND.

EE WAS TRIMMING A CONVEYOR BELT AND HIS UTILITY KNIFE SLIPPED AND CUT HIS INDEX FINGER ON HIS LEFT HAND.

EE was pulling a piece of conduit onto the continuous miner cable and his hand slipped and struck a piece of rock, resulting in a cut.

Flash burns (electric)

EE was energizing an electrical breaker and received flash burns to his left hand when the breaker was energized.

EE was lowering the head of the continuous miner on some crib blocks that were being used to block the head up. When pressure was applied to the crib blocks, one crib block twisted out and struck EE on the right jaw.

EE was operating a scoop hauling cable bolts. A cable bolt became fouled on the rib, sprang back and struck him on the left eye brow.

EE was installing shaft on crusher with slate bar, bar slipped and caught finger between slate bar and flange.

EE WAS UNHOOKING A CHAIN FROM THE TAILPIECE THAT WAS ATTACHED TO A SCOOP AND DID NOT GIVE THE SCOOP ENOUGH TIME TO COME TO A COMPLETE STOP AND GOT HIS FINGER CAUGHT IN THE CHAIN, RESULTING IN A CUT TO THE END OF HIS FINGER.

EE WAS MAKING A BELT SPLICE. AS HE STRUCK THE CRIMPING TOOL WITH THE HAMMER, A SMALL PIECE OF THE CRIMPING TOOL CAME OFF AND LODGED IN HIS LEFT ARM.

EE WAS CUTTING CABLE WHEN KNIFE LATCH BROKE AND CUT HIS LITTLE FINGER ON RIGHT HAND.

Fall onto or against objects

EMPLOYEE IN OFFICE STATES GOT UP WALKING FELT DIZZY M, REACHED FOR THE CHAIR , BUT FELL DUE TO DIZZINESS AND HIT MOUTH ON CHAIR, KNOCKED OUT TOOTH.

EE was throwing the flop gate in the prep plant. It became fouled, and he was shaking it with his left hand. When the flop gate freed, it caught his finger against a piece of angle iron, cutting it.

EE was cutting a strap that held a bundle of roof bolts together. When the strap came loose, it struck him on the left wrist, resulting in a cut.
