Employee was directing an end loader towards himself to hook a strap to it for removal of a dolly that was used to move the hi-wall miner launch. Contributing factors were attempting to hook a 6' strap while standing in front of this moving machine and stationary dolly. Cause was human error/bad judgement by the victim and end loader opr.

Pin was being removing from a pusher block on the highwall miner launch using a pry bar. He squatted down under the launch, pulled on the pry bar and felt discomfort in his groin. Cause was poor judgement in the means of removal of the pin. Rule compliance not a factor, mine equipment not a factor, proper clothing not a factor and miner proficiency and attitude may be a factor.
While walking from his personal vehicle to the parking lot, ee fell on the ice and struck his knee on the ground. There was a rock where he struck the ground. His knee struck that rock.
Setting up an hose to blow out filters, has he laid down the hose, the coupler came un-hooked striking him in the mouth (lip). Cause of this injury was not making sure coupler was fully coupled. Compliance with rules/regs was not a factor, personnel protective equip. not a factor, miner attitude and proficiency could be a factor, mining equip/systems not a factor.
