Employee was alone on the 3rd floor of the plant when he fell approximately 20 feet to the ground floor.

Fall from machine

Employee was dismounting a Cat D-9R dozer when he slipped and fell. *He did not start missing work until 12/10/07.

Struck against stationary object

Employee was removing hardware from power pole that was laying on the ground. He was pulling old ""buttground"" off pole with his hands. His hands slipped causing him to lose his balance. He stepped back and stepped on a nail that was in a board that was hidden by mud.
