The Tyro Creek elevator was down from 12:20 pm to 11:20 pm as a result of a power outage. When power was restored, Bagby elevator was called to make some adjustments & check-out the systems.
The Tyro Creek elevator was down from 10:30 pm on 10/17/06 to 1 am on 10/18/06 due to insufficient airgap on brake causing the brakes to overheat. Bagby was called & corrected the problem, tested & placed back in service.
The Tyro Creek Elevator was down from 3 pm to 7:15 pm due to the brakes being out of adjustment causing them to lock up. Bagby was called & repaired the problem & tested the elevator & everything checked out okay.
Cedar Creek elevator is out of service as of 9/13/06 due to electrical problems. Bagby Elevator Company examined the service elevator, but did not have the part needed to repair it. We are presently still in search of part to repair elevator.
A roof fall has occurred in the intersection of L Main entry #6 at spad 57 + 38. The fall measured 21' long x 19' wide and approx 8' thick.
The Cedar Creek Service Elevator was out of service on 7-31-06 from 9:00 pm until 11:15 pm due to slack in the braking system. Bagby Electric Company adjusted the braking system and the elevator was put back in service at 11:15 pm on 7-31-06.
The Tyro Creek elevator was down from 2:30 until 4:15am due to limit switch.

The cedar Creek Service Elevator was out of service on 7-19-06 from 8:45 pm to 9:40 am due to electrical problems. The cedar creek elevator was also out of service on 7/20/06 from 1:30 am to 4:30 am due to door limit switch problem. Bag by elevator co. was called and repaired both items
The typro creek service elevator was out of service on 7/17/06 from 10:47 pm until 7-18-06 at 1:40 am due to slack in braking system. Bag by elevator co. adjusted the braking system and then elevator was pit pack in service at 1:40 am on 7-18-06.
The Cedar Creek service elevator was out of service from approx 4:15 am to 8:00 am due to problems with power supply to drive.
A roof fall has occurred in our Rehab Section (MMU 017-0) in the old ""A"" main track entry. The roof fall measured 18' wide x 15' long x 30"" thick.
While going down the track, smoke was seen coming out from under the panels. The utility men used rock dust & fire extinguishers to put out the fire. It was determined that the fire started when exposed wires came into contact.
The Cedar Creek Service Elevator was out of service from 11:37pm on 4/1/06 to 1:20am on 4/2/06 due to the over speed tripping on the elevator in the up run mode. Bagby elevator company came and changed out they tacho-meter and tested the elevator and then it was put back in service.
A methane ignition occurred at drill platform over an 8 3/4 inch slurry injection hole leading into the sealed Gail panels.
The Cedar Creek Service Elevator was out of service on 2/22/06 from 1:20pm to 4:03pm due to the armature voltage signal not being picked up which did not set the brake all the way causing the car to drift into the overhead frame. Bagby elevator co. came and made repairs to the ""M"" contactor and then the elevator was put back into service.
The cedar Creek Service elevator was out of service on 2/9/06 from 6:55 pm until 10:00 pm due to trash in the truck for the doors and then they could reset the door operator. bagby elevator company came and made the repairs and then the elevator was put back into operation.
A roof fall has occurred in the crosscut between no. 5 & 6 entries of L-main set of entries at spad 9+85. The fall measured 7' high x 8' wide x 40' long approximately.
The Tyro Creek Service Elevator was out of service on 1/24/2006 from 6:15 am until 7:30am due to a defective limit switch. Bagby Elevator Co. replaced the switch and the elevator was back into operation.
A ROOF FALL HAS OCCURRED IN OUR REHAB SECTION (MMU #017-0) IN THE #4 ENTRY OF THE A MAIN DEVELOPMENTAL AREA, APPROXIMATELY 260 FT NORTH OF CEDAR CREEK ELEVATOR. THE FALL MEASURES APPR. 21 FT LONG X 16 FT WIDE X 8 FT HIGH.
Employee was helping remove an overflow pipe when his finger was caught between the pipe and the cyclone.
Employee was climbing down from a 5' scaffold using the scaffold rungs as steps. Loosing his footing, he fell approximately 3.5' to the ground.
Employee stated he was helping another employee put up racks in the storage building, the other employee fell. While helping him up the racks fell against the EE, he fell to the ground causing him to hit his left shoulder against the rail. His FLWD did not occur until Nov. 10, 2006.
Employee stated he was helping another ee install shelving at the prep plant when some of the boards he was standing on fell through. He has been diagnosed with cervical degeneration of the spine, arthritis and a cyst in his lt. shoulder that was aggravated by his fall. His first lost work day did not occur until 11/22/06.
Employee stated that he was repositioning a high voltage cable when he felt a pain in his shoulder. The employee's first lost work day was January 18, 2007 due to surgery.
Employee stated that he was washing on the slope. He stepped on a mandoor that gave way causing him to fall. He was diagnosed with a rotator cuff tear. His first last work day did not occur until 7/20/06 due to surgery.
Employee was assisting in re-socketing drag ropes on dragline#4. While removing the 3 piece wedges on the left socket. While removing the 3 piece wedge it slipped out of his left hand and fell approx. 8"" pinching his right distal middle finger between a piece of the wedge and the ground. Resulted in lacerations to finger and a possible fracture.
EE stated that while troubleshooting the forklift begam to tram. It spun to the left catching his foot between the forklifts frame & ground. His first lost work day did not occur until 4/27/06.
Employee was carrying fence posts & stepped in a hole.
Employee stated that he was throwing a track switch. He pushed the switch handle down and before he could release it, the switch arm came back up jamming his shoulder his first lost work day did not occur until 1/26/06.
EMPLOYEE STATED THAT HE WAS WALKING DOWN SLOPE TRACK WHEN HIS FOOT SLIPPED IN A DRAIN HOLE CAUSING HIM TO FALL. HE WAS DIAGNOSED WITH A LOWER LUMBAR STRAIN. HIS FIRST LOST WORKDAY DID NOT OCCUR UNTIL 1/19/06.
EE loading piece of broken buching onto forklift...piece slipped off fork & struck EE on left leg shin bone causing bruising & swelling.
EE was removing magnets from a tube going to a transmission screen. EE was pounding the tube down in order to remove magnets. While pulling a magnet from the tube a sharp edge was formed and the EE cut finger on the edge of the tube.
Employee stated that he was attempting to cut a plastic tie when the knife slipped, cutting his wrist. The laceration was closed with sutures.
EE stated that while releasing a belt clamp by lowering the belt clamp support jack, the belt clamp did not lower because it became ""hung up"". EE placed right hand under belt clamp and belt clamp ""freed"" and fell apprx 6"", striking EE's hand. This resulted in two sutures.
EE was changing a ripper tooth off a dozer. He hit the ripper tooth with a hammer, which knocked off a small piece of metal from the tooth, that hit the EE in the chin, requiring 2 stitches
Employee stated that he was moving a pile of ""gob"". A 3 inch pipe was in gob & was caught in a bind. The pipe turned loose & struck the employee in the jaw.
EMPLOYEE STATED THAT WHILE TAKING DOWN AN OVERCAST WALL A CONCRETE BLOCK FELL ON HIS ARM & HANDS. THIS RESULTED IN A FINGER FRACTURE AND SUTURES.
EMPLOYEE STATED THAT HE WAS GUIDING A ROLLER TO THE CATWALK WHEN THE ROLLER TIPPED OVER, CATCHING HIS FINGER BETWEEN THE ROLLER AND A PIPE. THIS RESULTED IN A FRACTURE OF THE RIGHT INDEX FINGER AND SUTURES.
EE STATED THAT HE WAS BOLTING THE ROOF WHEN A PIECE OF COAL FELL AND STRUCK HIM ON HIS RIGHT CHEEK. THIS CAUSED A LACERATION THAT REQUIRED 3 SUTURES.
Employee stated that while attempting to lift a jack bar, he caught his finger between bar and frame. This caused a laceration that required 2 sutures.
EE stated that he was cutting a piece of belt. His left hand slipped off of the knife and cut his left index finger. The laceration required 8 sutures.
Two employees were placing a 1'x9'x3/8"" piece of metal on side of a truck for welding. The injured employee was using a pry bar to adjust the piece of metal. He had his one hand behind the metal piece when the pry bar slipped. The large piece of metal moved and wedged employee's finger between two pieces of metal.
Had washed top floor of loadout, in freezing weather. As he was descending three step to lower level foot slipped on ice. Put left hand down to break fall. left little finger went through opening in floor grating and left 5th metacarpal was fractured.
EMPLOYEE STATED THAT DRILL STEEL HUNG IN ROOF. WHEN HE REACHED UP TO GRAB IT, IT FELL AND STRUCK HIS LEFT THUMB. IT CAUSED A LACERATION THAT REQUIRED SUTURES.
EE alleges that welding for 20 years has caused carpal tunnel syndrome. Surgery required.
EE has been diagnosed with bilateral Carpal Tunnel Syndrome (work related). He is currently undergoing conservative treatment, but he may require surgery at a later date.
