At approx. 2:30AM the service hoist lost power due to a rat chewing a lead in too an causing a ground. The hoist was down approx. 70 minutes. No persons were trapped or injured. All employees were evacuated to the surface in a timely manner.

All incidents in 2015
At approx. 10:00 PM the service hoist became inoperable due to a blown fuse causing a single phase. The hoist was inoperable for approx. 150 minutes. No persons injured or trapped. All employees were safely evacuated to the surface in a timely manner.
Elevator indicated a fault during a dry run. All employees were notified to evacuate and fault was cleared and elevator was inspected and back in service at approximately 10:10am on 8/24/15. No persons injured or trapped.
Service hoist became in-operational due to power glitches. Hoist was inoperative for approximately 90 minutes. No persons injured or trapped. All employees safely evacuated in a timely manner.
Hoist crew was attempting to remove a diesel motor from underground via the service hoist. Overhead service hoist located on the service hoist stopped working. No equipment damage or injuries occurred. Overhead service hoist stopped working which caused the service hoist unable to be used to transport persons. Hoist was out of service for approximately 60 minutes.
Escape elevator showed a fault during a weekly airway exam. Fault was trouble shooted and reset.
Guides malfunctioned during inspection, were readjusted.
Extremely cold temperatures causing faults in Emergency Elevator. Ice accumulation also.
An unplanned roof fall occurred on the feeder line of the #1 Unit in #8 entry intersection approximately 3 stopping lines outby the right LOB at survey spad 4383. Height of fall was approximately 4-7' and width and length was respectively 20x20'.
An examination of the emergency escape facility was being conducted and after the upper level over travel test, the hoist malfunctioned and would not operate. During the troubleshooting process it was discovered that a 24 VDC relay had a bad set of contacts and the top landing switch was not making contact properly.
Employee (bolter operator) was located in the #6 entry on the #1 Section (003 MMU)and was walking ahead of a fletcher RRII bolt machine and was struck by the bolt machine as it was tramming through ventilation curtains. Employee was positioned between the bolt machine and a Joy 1415 CM. Employee received multiple fractures to his pelvic area.
Employee was working on ventilation when his leg got tangled up in a piece of line curtain causing him to fall hurting his right knee. Note: employee returned back to work on 10-21-15 and worked until 2-29-16. On 2-29-16 he was placed off of work due to a medical evaluation.
The employee stood up from the kneeling position and felt a pain in his right knee. Employee was working on a rubber tired ride near the bottom diesel shop area. Employee finished his shift and returned to work. On 7/13/2015 doctors put him on restricted duty.
Employee was walking from service hoist to bath house and slipped and fell on his SCSR that was attached to his belt.
Employee was attempting to remove a jack to pull a loose rib that was dangered off. When jack came loose the rib fell striking the employee on the right leg. Hairline fracture to right leg.
Employee was installing a roof bolt when the bolt became stuck inside of the drill chuck. The employee was going to lower the drill head when he accidentally hit the rotation lever causing the bolt to strike his left wrist.
Caught finger between a chain and tail pc.
Rock bounced off slope hitting employee in his right hand which caused a laceration that required 11 stitches. Employee was treated and released and returned to work on his regular scheduled shift. NO DAYS MISSED
While handling a 5 ton lifting jack his right ring finger became stuck in the foot lift arm portion of the jack. Employee was wearing gloves but x-rays revealed his right ring finger was fractured.
Employee was using a jack to rerail a flat car but the jack bar slipped out of the jack catching the employee on the right pinky finger. Slight fracture to right pinky finger along with stitches to right pinky finger.
Employee was hooking up a down drill near a section tailpiece and slipped and fell to mine floor landing on his left wrist. Caused a hairline fracture to his left wrist.
Employee (mechanic) was repairing floor jack and while trying to dislodge a small chisel (approximately 6"" length) he was struck in the mouth with the chisel. Received 7 stitches to his lip.
Employee was entering D8T dozer, as he turned his footing slipped and fell on his right wrist. Right wrist was fractured.
Drill steel was stuck together and while employee was pulling the steel apart it broke loose causing him to strike himself in the nose with his hand and drill steel.
Contract employee was finishing a job and while cutting belt with a utility knife his left was holding a piece of belt and his handing holding the utility knife slipped and cut his left hand below the thumb and palm area.
Employee was trying to straighten a bolt when his hand slipped off the bolt and employee's body fell forward and he caught himself with left hand causing a sprain to his left wrist. Received a wrist wrap from doctor and returned to his regular duties the following shift.
Employee was trying to remove a rock from the ATRS and as he shoved it off the ATRS his left hand was pinched between the top of the rock and the mine roof as it fell forward towards the face.
Employee was using a scoop to knock an old stopping and while knocking the stopping an object struck him in the head. Received stitches and returned to work.
Employee was attempting to change batteries on a scoop while working outby and the battery plug fell and struck his right middle finger. Employee finished his shift and received x-rays two days later that revealed the middle finger was broke.
