Fall from machine

Employee was exiting the cab of the drill. He stepped down on the 1st step, then missed the 2nd step & fell about 4', landing on the ground. He stated that he was unable to grab the handle in time to keep him from falling. Other than failure to maintain 3-point contact, there appear to be no contributing factors causing the incident (moisture, wind, defective equipment, etc.).

Struck against stationary object

Technician was installing cover on air conditioning fan when his hand slipped and his finger hit the fan. Did not receive stiches, but had skin graft later.
