Accident type, without injuries

A unintentional roof fall has occured @ xc-28 on 2mw between travelway and return. Length 20', width 17', Height 4'.

Struck against a moving object

At approx 5:20 pm a 40 ton articulating truck was traveling along a haulage road impacting a 63"" high berm. The truck traveled across the berm and fell 72 feet to the mine floor below. The victim was extracted from the truck by EMS and airlifted to the Hospital.

Pulling up slack to hang miner cable, when foot slipped and pulled something in left calf.

Examining Section, stepped into a hole and twisted left knee.

Traveling inby on 3rd NW Travelway @ xc-12 driver's foot became wedged between gas and brake pedal, pinching employee's left ankle between mantrip and rib.

Walking outby between rib and line curtain, draw rock fell, striking left cheek bone (stitches required).
