Fall from ladders

Employee was descending the ladder, got to the third step and missed a step. He then slid down the rest of the ladder and hit the ground and rolled onto his belly, during this time he twisted/strained his back. He remained on his belly until ambulance personnel arrived and placed him on a backboard. He had been up and down the ladder several times that morning.

Fall onto or against objects

Employee was installing big electrical boxes in a box when the straps securing the box slipped. He tried to jump from trailer to keep from being pinned by the box. When he jumped he landed on his side across a starter box which was on the ground below box trailer. When he landed he injured his rib cage and was taken to E.R.
