The employee was using a slate bar to pry the clean coal crusher door closed while another employee used a crib block to hit the door. The slate bar slipped and mashed the right hand of the employee and twisted his lower back.
The employee was closing the doors on a dryer and he grabbed the latch to latch the door when his hand slipped on the latch cutting his left hand under his left index finger. The latch had a sharp edge on it.
Employee stated he twisted his ankle as he was stepping onto the ground from the skid-steer
Employee stated he became ill from possible heat exhaustion.
Employee stated while positioning a feeder motor for installation, employee felt pain in his lower back.
Employee stated that while getting on a 988-G Front-End Loader, he slipped on the first step and while trying to grasp the handrail he pulled his shoulder.
EE states while using an End Loader and chain to set grizzly (sizer) in place over Coal Feeder injured unhooked the chain motioned the End Loader operator to back up, when the loader operator engaged in reverse the E.L. rolled forward pinning EE's left foot between the bucket and the Grizzly.
Employee stated: I was upset after a co-worker was injured. I let out my frustration by striking the outside wall of the office trailer. This is a self inflicted injury and will not be a lost time.
Employee stated while picking up a piece of malloy (1 inch thick plastic) weighing approx 25 to 30 lbs, employee felt pain between shoulders (this will be a lost time accident).
EE stated, while attempting to take a measurement to replace a section of pipe, injured placed his hand on the pipe causing the pipe to fall striking the EE in the back and rt ankle. (This will be a lost time)
Employee states he was spraying tracking switches w/PB Blaster on belt press #2 and, while removing mud from switch, bumped switch which allowed air to blow PB Blaster into his left eye. Employee received medical treatment, was taken off work by eye care physician for one day & returned to work full duty on next scheduled day.
Employee was using a utility knife to open a box of welding rods when the knife came apart sending the blade flying into employee's right thigh, cutting the employee. Medical treatment was sought and the employee received 3 stitches. There is no lost time associated with this injury.
Slipped on metal stairs & fell. Landed on right elbow and jammed right shoulder also. Elbow swollen. Shoulder sore.
Employee stated while washing around caustic tank with water hose, chemical splashed on his left arm on 7/14/11. Employee did not report injury until 7/13/11 after obtaining medical treatment. Employee lost 4 days work and returned on 8/8/11.
Employee states he was opening a valve on the water sump, and the next morning, had pain in his right side.
EE stated, I pulled a muscle in lower back, picking up 55 gal. drum. Emptying into a Bob-Cat bucket. Used hands to almost empty barrel. Some scrap metal left in barrel , lifted barrel and heard popping noise in lower back.
Employee was working in unnatural position and object entered eye under safety glasses. Medical treatment was received, requiring prescription eye drops.
EE states he stepped off a skid steer onto uneven ground, twisting his right ankle and causing muscle strain. EE was placed on modified duty 12/9/10 and released to full duty on 12/16/10. There is no lost time associated with this injury.
EE states a co-worker was pulling a wheel barrow up the stairs when the wheel barrow slipped out of the co-workers hand the EE fell on his knee getting out of the way. Medical treatment was given and the EE missed one day and RTW to full duty on 5/31/10.
Employee stepped off Cogar feeder onto loose material and twisted right knee. Employee suffered muscle strain and was placed on restricted duty.
Employee states that he slipped and fell approx. 4 ft. from the top of a dryer as he was descending, landing on his back and left elbow.
The employee stated he was climbing up the ladder on the front end loader to begin his shift when his foot slipped causing him to catch his weight with his arms causing pain in his chest.
Employee stated he fell while descending steps & hurt his right arm by straining it. Employee was bagging trash & taking it out of the office when he descended the steps. He was terminated for other reasoning, left the site & came back hours later and told the supervisor he had an accident before being terminated. Employee was splicing two pieces of 6" pipe and experienced popping in lowr back when he lifted piece of pipe.
Employee got a piece of weld slag in eye (right) while chipping at metal after welding.
Employee lifted a 30 ft fiberglass extension ladder and suffered lower back pain.
Employee was welding in reclaim tunnel & suffered a corneal abrasion in left eye when weld slag entered his l eye under welding hood.
EE was walking from one area of the site to another, slipped on concrete form pins and fell on his right shouldr, fracturing it.
