The employee went beyond a roller guard that was put there to prevent anyone from reaching the roller. They should not have done that.
While removing back plate covering screen, after taking nuts and washers off we did not know that the bolts were not welded on inside of screen. The weight of the plate caused the bolts to angle downward and the plate to slide off bolts smashing employee's left hand.
