On said date and time miner was operating loader 35385 when the a/c quit working. EE stayed in that machine for another 20 minutes in which EE reported this to MGR. EE had another loader with a working a/c to use. MGR came to mine site and took EE to Urgent care where EE was treated and released to full duty.

Showing all 3Had the employee used the tools that were provided and on hand the employee could have washed the machine from the ground. This would have stopped the need for climbing up on the machine needing fall protection, 3 points of contact and most of all stopped the employee from falling and injuring self.
STEPPED IN MUD AND SLIPPED FELL TO KNEES. DID NOT REALIZED THAT SHE HAD HURT HER BACK TIL THE FOLLOWING SUNDAY 1-2-03 SHE WAS AT WORK MONDAY 1-13-02 SHE WAS OFF TUESDAY AND WEDNESDAY. BUT DID COME IN ON WEDNESDAY 1-15-02 LONG ENOUGH TO REPORT THE INJURY TO ME. I WAS NOT IN THE OFFICE SO SHE REPORTED IT BY PHONE. SHE SAID SHE HAD PULLED SOME LOWER BACK MUSCLES.
