Employee was cutting a rubber matt with a utility knife and knife slipped and cut the top side of EE's left arm by EE's wrist. required stitches
Tightening truss cable on a conveyor with a turn buckle and the turn buckle broke and cut his hand.
They were putting belt on conveyor. Chain came off of bar holding belt. Belt swung over and hit employee who was on a ladder and tipped him and the ladder over. Broke his left arm

We were putting in a tile, I dug the hole about 7'wide 7' deep. We were working on the 2nd half and a shovel fell in. EE said he would get it, I told him no the bank needed cut wider. As I was doing that he went in to get it I yelled for him to stop as the bank was caving. He tried to run but didn't make it.
EE was cutting a conveyor belt and the utility knife slipped and went into his leg just to the side and above the knee. EE was cutting belt in the rain. EE received 2 stitches.
Was running backhoe, got something in his right eye.
Lots of rain, muddy and wet.
Employee was taking the belly pan off 250 E. to wash. Was on his knee's with impact in right hand at arms length above his head. When bolt came loose pan fell and pulled his back trying to get impact out.
The wind blew dust and dirt off of the conveyor and got something in EE's eye. Doctor removed particles from upper eye lid.
Employee was working down by the crusher & it was real windy and got something in his eye. Went to Doctor on 11-1-06. Doctor took a piece out of the top of his left eye & put him on eye drops.
Loosened bolts to drop jack. Jack fell by itself and hit on left shoulder see attached as to why not reported til now (Went to dr 7/24/06 for x ray-negative, mri 7/31/06 with result, physical therapy because he was still having problem with his left shoulder. Therapy start 8/21/06, go back to dr 9/14/06. Accident did not report until dr ordered physical thearpy)
EE was climbing down off the dragline & when he stepped down the ground was uneven he fell. Broke his right leg & has a chip on his right ankle.
EMPLOYEE FLIPPED AND FELL OFF CRUSHER. HE CRACKED HIS PELVIS.
Employee was getting out of the 250 dump truck. He stepped down and twisted his left ankle then heard it pop.

COWORKER WAS RUNNING LOADER. INJURED EE WAS HOOKING TOUNGUE OF CRUSHER INTO THE HITCH ON THE 966C FRONT END LOADER. LOADER PINCHED RIGHT END RING FINGER. THE TIP WAS CUT BADLY.
