Miner was pulling a jackleg from a hole and bumped EE's elbow on a Monster Mat on the rib. Causing a laceration that was 2.5 inches and required 6 stitches. Miner returned to work.

A miner was bolting the back of the 833 N stope. A fall of ground in the hanging wall fault zone buried the miner until the miner could be rescued by other miners on the crew. The miner received a broken jaw, broken femur, broken rib, fractured hip, several face abrasions, as well as missing teeth.

Miner was drilling and noticed his nipper had collapsed behind him. There was no apparent reason for the collapse such as getting hit by a rock or the drill leg, etc. Miner transported nipper to the surface, notified emergency personnel, and nipper was transported to Kootenai Medical Center in Coeur d'Alene, Idaho.

EE was inserting a split-set bolt in the rib when a loose piece of quartz from the vein (back) fell onto his hard hat and cut and scratched his face. The vein is highly oxidized and fractured near a fault.

Employee was sitting on rear of jumbo drill as it was drilling holes at the face. He jumped off the jumbo a vertical distance of about one foot to get fuel for the jumbo and landed on the side of his right foot. The right foot became immediately painful, but he finished out his shift to 4:00 pm.
