EE was working with a crow-bar and while he was pulling back the crow-bar unlatched and hit him in the face.
Hearing test showed possible standard threshold shift
EE was walking in the quarry and stepped on a small rock. The area was well lit and no hazardous conditions existed. EE did not think anything of it; so he kept working. At the end of his shift (approx 8 hours later) he noticed his ankle was swollen and told his supervisor. He told ee he needed to see a doctor. The office manager helped ee fill out the paperwork.
Employee slipped & fell from dozer track.
The employee was working inside the hopper of a jaw crusher on a 6' ladder when he fell. The employee was torquing the bolt on the grizzly when the wrench slipped of the bolt and he fell. He was wearing his hard hat, but hit his head, back and arms and his hard hat fell off. He indicated that he passed out for couple of minutes.
Employee complained of back pain and muscle pain. Was diagnosed with back strain, groin pull and hernia. Did not report accident until 2/27/06. Employee could not provide cause of injury.
Employee was walking down stairs when she slipped on the landing and twisted her ankle.

Employee returned to work after his normal 2 days off. About 9:00 pm he first complained of general pain & exhaustion. His team leader told him to rest for a few minutes & consume some water. At 10:30 pm he complained about pain & exhaustion & was told to go home. At 10:45 pm he collapsed & 911 was called. Paramedics transported him to the hospital at 11:05 pm.
The employee left at the end of his scheduled shift. After returning home he felt weak and nauseous and went to the hospital, where he was diagnosed with dehydration.
Employee tried to use a shovel to free a stuck return roller. The shovel stuck in the roller and the handle struck the employee's right thumb.
While driving the waterpull to water roads, the employee experienced extreme bouncing and jerking around which caused lower back pain
Employee was pressure washing the floor of the clarifier when the spray bounced back and material entered his eye.
EE WAS CLIMBING OUT OF THE 980G FRONT END LOADER TO CHECK THE CRUSHER. AS HE WAS STEPPING FROM THE LAST RUNG OF THE LOADER LADDER, HE STEPPED ONTO A ROCK, TWISTING HIS ANKLE RESULTING IN A BREAK OF HIS LEFT ANKLE.
EE WAS HELPING LIFT CABLE INTO GEN-SET (HEAVY WIRE) WHEN HE FELT PAIN IN HIS LOWER LEFT STOMACH.SURGERY PENDING.
EE WAS RESETTING A FORM FOR CONCRETE, WHEN HE STARTED DRIVING A STAKE WITH THE HAMMER WHEN HE FELT SOME PAIN IN HIS LOWER BACK.
EE WAS WORKING ON LOADER BUCKET W/SLIGHT BREEZE AT THE TIME. HE WAS FINISHING THE JOB WHEN HIS LEFT EYE BECAME IRRITATED & STARTED TO ITCH. AFTER HE COMPLETED THE JOB, HIS EYE WAS STILL ITCHIN G AS HE PROCEEDED TO THE EYE WASH STATION TO FLUSH HIS LEFT EYE. HIS EYE CONTINUED TO ITCH THE FOLLOWING DAY, SO WE DECIDED TO SEND HIM TO THE URGENT CAR/EYE DOCTOR.
EE WAS PUTTING A CUTTING EDGE IN THE LOADER BUCKET TO MOVE IT TO A STRONG LOCATION
AS I WAS WALKING DOWN THE STAIRS OF THE PARTS TRAILER (OTC) MY LEFT ANKLE GAVE OUT AND I SAT DOWN ON THE STAIRS. IT HURT AND STARTED TO SWELL ALMOST IMMEDIATELY.
THE EMPLOYEE WAS SPRAYING DRESSING ONTO THE HEAD PULLEY OF A CONVEYOR DUE TO SLIPPAGE. AFTER AN HOUR AND FORTY FIVE MINUTES, HE COMPLAINED OF A HEADACH AND A SORE THROAT. HE WAS THEN TAKEN TO THE HOSPITAL FOR A CHECKUP.
WHILE CENTERING THE TAIL SHAFT & TIGHTENING TAPER LOCK ON RIGHT SIDE OF TAIL SHAFT - RATCHET SLIPPED, SMASHING THE RIGHT RING FINGER BETWEEN THE TAIL PULLEY & RATCHET HANDLE. THERE WAS ALSO A TURF FRACTURE TO THE TIP OF THE FINGER BONE.
WHILE CLEANING OUT GRIZZLY FINGERS IN JAW CRUSHER, HE SMASHED HIS HAND WITH SLEDGE HAMMER.
EE WAS IN THE PROCESS OF PULLING SLACK OFF OF THE FINES BELT OF THE 391 PORTABLE PLANT. AS HE WAS COMPLETING THIS TASK, A PIECE OF ANGLE IRON WAS BEING HOISTED AT THE SAME TIME. AS EE WAS MOVI NG OUT OF THE WAY OF THE STEEL PLACEMENT, IT CAME LOOSE FROM THE STRAPS STRIKING EE ON THE RIGHT FOREARM. IT CAUSED SOME MINOR BRUISING, A SCRATCH AND SOME SWELLING. HE WAS TAKEN TO THE ER FO
RADIATOR CAME DOWN AND BOLT STICKING OUT, HIT HEAD AND CUT OPEN.
EE WAS TIGHTENING A BOLT WITH A RATCHET. HE LOST HIS FOOTING AND WENT OVER HEAD FIRST DOWN ABOUT A 10' DROP. WHILE FALLING HE HIT HIS RIGHT KNEE ON STEEL AND SLAMMED JAW ON TO A ROCK AND TWIST ED HIS WRIST. TAKEN TO HOSPITAL.
