Showing all 6Maintenance employee was replacing a skirt rubber on a chute above the main feed belt. The employee did not Lock Out the feed belt and while working on the chute a piece of metal fell onto the moving belt, the employee tried to grab the metal and caught his right arm between the belt and belt cover. The employee sustained a fracture to his right wrist and forearm.
