EE was operating a Cat 988F end loader. Another end loader came around small pile of coal and struck the rear of the other loader.

EE was attempting to change the position of the flopgate at the head of the #2 raw coal belt. The flopgate had become wedged against a small bolt head inside of the chute.While using a prybar to dislodge the gate the bar slipped out of the EE's hands. He grabbed the bar as it fell and wrenched his shoulder. EE started missing work on 5/11/10.

Employee had parked a Cat 938 at the service dock for service. He decided to clean his front glass & stepped out onto the top of the wheel fender. As he finished, he grabbed the top of the light housing on the fender & it broke. He jumped to the ground when he lost his balance. Originally a knee sprain. Did not miss work until 7/7/10 for arthoscopic surgery

EE was operating a gob truck in the pit area. He was on his way out of the pit, hauling shale back to the gob dump at the Prep Plant, when he ran over a hole in the haul road causing him to go up and down very suddenly. EE did not start missing work until March 19,08. It was determined by the doctor he had trauma to the abdominal area.

Employee stated that he was mounting his Cat-777 end dump using the factory steps and hand rails. As he was reaching for the hand rail beneath the window of the cab (while standing on the 3" wide step) he lost his balance, spun around, and fell to the frozen ground.

Employee was attempting to remove bolt and nut from a flange coupling on a 6" feed line to belt press. He had a long ratchet w/ socket attached on one side and a 15/16" combinations wrench attached on the other side. The bolt and nut was seized (frozen), the ratchet slipped causing the wrench to move pinching his little finger between the wrench and the flange of the coupling.

After the completion of the investigation on April 10, 2007, it was determined that I was responsible for reporting the injury. Another vendor (salesman) was walking through the prep plant on the third floor where my employees where working and had removed a piece of grading to conduct some repairs on a I-beam and EE stepped into the hole.

Employee stated he had stomach cramps the week before. He had also tried to install a window air conditioner at home prior to the start of his shift. He thought he had drunk enough water. He felt a little faint after his shift started. He came out from under equipment and was kneeling when he felt faint again and lay down.

EMPLOYEE IS STATING HE WAS STEPPING BACK FROM FUELING A DOZER TO THE CONTAINMENT STRUCTURE AROUND A DIESEL FUEL TANK. HE LOST HIS BALANCE AND STEPPED DOWN TO A PLATFORM (APPROX 2') PUTTING HIS WEIGHT ON ONE LEG. THIS ACTION CAUSED AN INFLAMATION TO THE OSTEOARTHRITIC CONDITION OF HIS LUMBOSACRAL SPINE THAT HAS EXISTED FOR A PERIOD OF TIME.

EMPLOYEE IS STATING THAT HE WAS HELPING CHANGE HYDRAULIC LINE ON THE SHOVEL BOOM. HE IS STATING THAT HE SLIPPED ON BOOM STEPS & CAUGHT HIMSELF BY GRABBING A BANK OF HYDRAULIC HOSES & WRENCHED HIS RT. SHOULDER. EE IS STATING HE WORKED THE REST OF HIS SHIFT & PART OF THE NEXT SHIFT ON 8/3/04, HE THEN FELT HIS SHOULDER "POP" ON 8/3/04.

Employee is stating that he felt back pain on 4-16 and 4-21 while performing duties as a deckhand on a coal barge. Employee sought medical attention on 4-21. Ee had degenerative spine disease.

EE IS STATING THAT HE WAS OPERATING THE HITACHI EX1800 SHOVEL IN THE WEST END OF THE AREA #2 PIT STRIPPING SHALE AND LOADING END DUMPS. HE BACKED THE MACHINE UP AWAY FROM THE HIGHWALL TO ALLOW A DOZER TO CLEAN THE BOTTOM. AS HE STARTED TO MOVE UP A SECTION OF THE HIGHWALL FELL IN STRIKING THE CAB AND MACHINE.

EMPLOYEE BECAME AGGRAVATED AT PLASTIC SEAT COVER AND USED HIS POCKET KNIFE TO CUT IT OFF. WHILE DOING SO HE CUT HIS HAND WHICH REQUIRED SUTURES. PLASTIC SEAT COVER WAS INSTALLED 3 YEARS AGO BE CAUSE EMPLOYEE HAD EXCESSIVE BODY ODOR. COMPANY PURCHASED AIR CUSHION TO HELP BUT EMPLOYEE DIDN'T LIKE PLASTIC.

EE IS STATING HE AND ANOTHER EE WERE WORKING TO CHANGE ONE OF THE TRACKS ON A D11 CAT DOZER. INJURED EE WAS STRUCK BY METAL FRAGMENT IN THE CHEST AREA WHEN THE OTHER EE STRUCK THE MASTER LINK WITH A 17# HAMMER. EE WENT TO ER ON 5-2-03, SENTTO A SURGEON PM 5/5 AND HAD METAL PIECE REMOVED ON 5-6-03

EE WAS REMOVING BOLTS WITH WRENCH STRAINED HAND.

EE IS STATING THAT HE WAS PUSHING A WHEELBARROW ON THE CONCRETE FLOOR OF THE PLANT WITH APPROX 10 #S OF LOOSE COAL/MUD IN IT. HE CROUCHED DOWN A LITTLE TO GO UNDER A STRUCTURE AND WHEN HE PROC EEDED HE FELT A SHARP PAIN IN HIS LOWER BACK. THIS CAUSED HIM TO GO TO HIS KNEES AND THEN ON THE FLOOR.

HE WAS GETTING INTO BULK TRUCK & FOOT SLIPPED ON FUEL TANK, HE HIT HIS LEFT KNEE ON FUEL TANK.

EE OVERFILLED A/C GAS TANK, SPILLED GASOLINE RAN DOWN INSIDE ENGINE COVERS. HE SHORTED OUT STARTER TO MAKE ENGINE START THERE WAS A SPARK, IT IGNITED THE GASOLINE, BURNING HIM ON THE ARMS AND FACE.

EE WAS HOLDING A PIN TO BE KNOCKED OUT, WHEN THE MECHANIC HIT HIM IN THE HAND WITH THE HAMMER.

WHILE TRYING TO LIGHT A CUTTING TORCH, THE OXYGEN HOSE BLEW.

EE WAS EMPTYING GOB BIN MUD WAS FLYING & CAME INTO SIDE OF SAFETY GLASSES & IN LEFT EYE

EMPLOYEE WAS OBSERVED SITTING IN THE 988 FRONT- END LOADER NOT MOVING AND UNRESPONSIVE. CPR WAS STARTED IMMEDIATELY AND PROCEEDED UNTIL EMPLOYEEWAS PRONOUNCED DEAD AT THE HOSPITAL AT 15:15 ON MONDAY, NOV. 12, 2001.

EMPLOYEE WAS REELING IN A FUEL HOSE WHEN THE FUEL NOZZLE STRUCK HIM IN THE LEFT LEG CAUSING A LACERATION. THE FIRST DAY HE MISSED WAS 12-17-01.

EE WAS HELPING CHANGE OUT THE SKIRT RUBBER IN THE #8 BELT TAILPIECE. HE WAS USING A PARY BAR TO PULL BELT BACK, WHEN BAR SLIPPED CAUSING HIM TO FELL OFF THE BELT. HE FELL APPROX. FIVE AND ONE HALF FEET TO THE CONCRETE FLOOR.

EMPLOYEE CLIMBED ONTO THE BACK BUMPER OF SERVICE TRUCK. HE SLIPPED AND FELL ONTO THE WASTE OIL BARRELL CUTTING HIS ARM.

WHILE OPERATING A GRINDER, EE WAS WEARING HIS SAFETY GLASSES, WHEN A PIECE OF METAL WENT INTO HIS EYE. (RIGHT EYE)

WHILE OPENING A 988B ENDLOADER FEEDING COAL INTO THE TRUCK HOPPERS, EMPLOYEE EXPERIENCED BACK PAIN.
