Operating dozer. EE was back-dragging the road at valleyfill #3 when he backed over a rock. EE said he didn't see the rock. When the dozer dropped down off the rock, EE said he had whiplash of the neck.

EE was pushing overburden in shot and a rock rolled into his run behind him, while he was backing up to make another push, He ran over the rock because he said he didnt see it, causing him to be shook violently.

EE parked his drill very close to the highwall, we assume to mark the drill holes. When getting back into the drill, he slipped and fell approx. 75' off the highwall. He broke his femur and has massive injuries to his left eye. His condition is still being evaluated.

EE was drilling & needed to rack dirt off of shot hole. He got the hoe attached to the back of drill, pulling it to the front. It got hung up; he pulled harder, straining the muscles in his neck.

Using the Jack-Hammer to break up rocks stuck in the rock crusher the jack-hammer slipped between the two rocks and pinned finger between the hammer and the rocks.

Propped ladder up on 5110 track to climb on to service at 5:00 am. Ladder slid in mud lost footing hitting right chin and knee on track causing knot and swelling approx. 1 hr later contributing factor, mud, oil, grease, and water.

EE WAS POUNDING A PIN IN PLACE ON THE DRILL. HE WAS USING A SLEDGE HAMMER TO DO THIS. HE HIT HIS MIDDLE FINGER ON HIS RIGHT HAND WITH THE SLEDGE HAMMER, SMASHING THE END OF HIS FINGER.

While helping mechanic, emp. spilled Mineral Spirits on his right thigh, he did nothing at that time, 1 hour later he washed the area, but it had burned his skin by this time.

Employee said he opened the radiator cap on D11R dozer before letting it cool down enough, hot fluids spewed causing 1st and 2nd degree burns on his right arm, the right side of his chest and the right side of his face under his eye.

Injured and two other mechanics were steam cleaning the swing motor on the PC650. They had to remove a panel of PC650 to get to the swing motor which they placed on hood of PC650. The panel then fell off hood and hit employee in calf of right leg. He was transported by ambulance to Logan Regional Medical Center.

EE was moving the 6" water pump at pond 3, he was talking the discharge hose loose, it stuck he pulled hard on it. causing it to come loose at once causing him to fall backwards with it, he hurt his left hand, left leg and ankle. said it was just sore and did not want to see a dr. he told the mine manager about it the next evening at approx. 4:30 pm.

EE was finishing for the day, while leavihng job site, came around curve he announced on cb where he was located a coal truck was coming up the hill at curvey #4. ee got stopped but coal truck could nt stop and hit ee vehicle head on causing ee airbag to deplay him in the nose the right leg on the dash.

WET BAGS OFF THE TRACK AND CARRIED THEM OVER TO THE DRILLED HOLES, HE SAID; THAT HE HAD SET ONE OF THE BAGS DOWN TO DUMP THE OTHER BAG AND WHEN HE TURNED TO PICK-UP THE 2ND BAG AFTER DUMPING THE 1ST HE HAD A SHARP PAIN GO UP HIS BACK. DAMAGE TO HIS BACK UNKNOWN AS OF YET.

STEPS WERE INTACT AS IN NEW FACTORY CON DITION. HUMAN ERROR APPEARS TO BE THE CAUSE OF THIS INJURY IN THAT AS LATE AS 7/22/02 SLIPS AND FALLS/3 POINTS OF CONTACT WAS OUT MONDAY MORNING SAFETY MEETING FOCUS.

EE SAID THAT HE TIRNED TO THE RIGHT WHILE SITTING IN HIS DOZER TO LOOK OUT THE DOZER WINDOW AND THAT HE BEGAN HAVING PAIN IN HIS LOWER BACK, HE HAD BEEN OPERATING THE D10 FOR ABOUT AN HOUR, WITHOUT ANY PREVIOUS DISCOMFORT. HOSPITAL TREATED HIM FOR MUSCLE SPASM AND GAVE HIM A MUSCLE RELAXER AND TOLD HIM TO COME BACK TO WORK ON MON 7/22/02.
