Incident occurred at night with light rain condition. Employee's foot slipped while climbing wheeled loader ladder. Employee strained right shoulder when he started to fall. Employee worked with some pain from 3/23/10 until 4/12/10. On 4/12/10 he was seen by medical service, diagnosis rotator cuff strain. on light duty 4/12/10.

Showing all 13An employee sustained injuries when he was cutting a scrap grizzly. The employee was struck on the side of his head (fractured bone) when a section of steel he was cutting de-stressed and pivoted upward with the sideboard. Another section of steel landed on the top of both of his feet. (both feet had lacerations). More detail to be ammended to this report.
On the day of the injury, the employee was at the site early to start equipment up for the start of the shift. He had just started the D5 Dozer and was attempting to egress it, when his right foot slipped off of the equipment provided step. His left foot made contact with the ground and twisted, as a result, a bone in his left ankle was broken. (sprained not broken per MM)
Clear, dark conditions: got loaded with coal and had a problem with the tarp. The tarp is manually operated and the driver climbed up to untangle the tarp rope. When the tarp untangled, driver lost his balance and fell against the trailer in the coal. No initial medical treatment but placed on restricted duty on 1/7/08.
Cutting a piece of rope and utility knife slipped and cut hand.
The employee reported that he had been working on a silt pan. He exited the pan by climbing over the side. Upon exiting he felt a strain in his back. On 11/21/05 employee visited a physician who believed he had a pinched cyatic nerve. It wasn't reported until 11/21/05
BEFORE DAWN, BUT IN A LIGHTED AREA WITH COOL TEMPS. AND LIGHT RAIN. AFTER THE WALKAROUND INSPECTION THE OP. MOPUNTED LADDER AND OPEN CAB DOOR. AS DOOR OPENED OP. SWUNG BACK AND SLIPPED OFF THE LOWEST STEP. HE FELT SOME PAIN IN HIS RIGHT FOOT.
CLOUDY,FREEZING RAIN CONDITIONS. WHILE EE WAS CLEANING THE WINDSHIELD ON THE DRAGLINE (STANDING ON THE DECK) WHEN HE TURNED TO HIS LEFT USING 3 POINT CONTROL TO GET IT MORE PAPER TOWLES USING LEFT FOOT SLIPPED OUT FROM UNDER HIM & FELL ON HIS LEFT SIDE BREAKING HIS LEFT HAND
THIS INCIDENT WAS NOT REPORTED BY THE EE UNTIL 2/6/01. EE WAS COMING DOWN A FIXED LADDER WHEN HIS HANDS SLIPPED OFF LADDER & FELL BACKWARDS 2 FTFROM GROUND AND LANDED ON RIGHT HIP. EE DID NOT REPORT INCIDENT UNTIL 2/6/01 WHEN EE STARTED TO HAVE BACK PAIN. AT THIS TIME INVESTIGATIONS IS BEING CONDUCTED TO DETERMINE WORK RELATED
EMPLOYEE HAS BEEN EMPLOYED WITH PG&E OPERATING SERVICES COMPANY FOR FIVE YEARS AS A EQUIPMENT OPERATOR AND HAS BEEN AN EQUIPMENT OPERATOR FOR 18 YEARS. EMPLOYEE UNDERWENT CARPAL TUNNEL SURGER Y TO HIS RIGHT HAND ON 1-09-01. AN INVESTIGATIONHAS BEEN STARTED. THIS HAS NOT BEEN DETERMINED WORK RELATED AT THIS TIME.
A DRAGLINE BUCKET WAS LAID ON ITS SIDE TO BE WELDED ON; EMPLOYEE MOVED THE BUCKET CHAINS AND FELT A PAIN IN HIS LOWER BACK AS HE BENT OVER TO ENTER THE BUCKET WORK AREA.
EMPLOYEE WAS HOLDING A NUT WITH HIS RIGHT HAND, ANOTHER EMPLOYEE WAS TIGHTENING THE BOLT WITH ANIMPACT WRENCH. THE NUT DISLODGED FROM A FIXED BAR, THE BOLT AND NUT TURNED PINCHING THE EMPLOYEE 'S RIGHT INDEX FINGER OFF BETWEEN THE NAIL BED AND THE FIRST KNUCKLE.
EE OPENED THE DOOR TO THE OFFICE TRAILER AND STEPPED ONTO THE PLATFORM. HE STARTED TO CLOSE DOOR AND SLIPPED ON ICE AND FELL DOWN THE STAIRS, FEET FIRST. THE EE HIT HIS HEAD, RIGHT ELBOW AND R IGHT RIBS ON THE BACK ON THE WOODEN STAIRS.
