EE was getting clogs out of the hopper when he stepped off into a hole and twisted his ankle. Which resulted in him having a sprained ankle.
Cleaning under belt with shovel, he pulled a muscle in his shoulder.
EE HAD CAN OD STARTED FLUID RESTING ON TOP OF HEATER UNIT IN CAB OF 966D LOADER. HEATER UNIT WAS ON. THE CAN RUBBED WIRES FOR HEATER CAUSING SHORT THAT IGNITED CONTENTS OF CAN CAUSING FIRE IN THE CAB OF LOADER.
EE WAS CHECKING VOLTAGE ON POWER TO FEEDER MOTOR. WHEN HE MADE CONTACT, A SPARK FLEW FOLLOWED BY THEM BLOWING OUT, CAUSING BURNS TO BOTH ARMS.
EMP REMOVING LOOSE MATERIAL UNDERNEATH BIN WITH SHOVEL.
WE HAVE OWENED RIVERSIDE GRAVEL SINCE 1987. HE IS CLAIMING HEARING LOSS FOR THE TIME HE HAS WORKED IN THE GRAVEL BUSINESS.
SLIPPED OFF 6 FOOT SHOULD NOT BE ON PIPE FOR ANY REASON. ON THE OTHER SIDE OF CONEYOR PIPE WAS UNDER GROUND.
