Employee was drilling hole steel stopped up, dropped head, steel stayed in hole, Ee put left hand on head to see if he had suction finishing steel fell out of hole, striking little finger on left hand against head.

While installing and adjusting scraper, metal dropped down and mashed end of finger.

EE WAS PUSHING BELT AND UNROLLING BELT WHEN HE HEARD SOMETHING LIKE KNUCKLES CRUNCHING IN HIS BACK.

EE WAS TAKING BAND OFF OF ROOFBOLTS. BAND HIT HIS HAND & CUT IT.

REPAIRING SCOOP, CUT THUMB ON PIN ON SCOOP.

EMPLOYEE WAS PULLING CABLE.

THE EE WAS HELPING GET A SCOOP THAT WAS STUCK. THE SCOOP SET DOWN ON HIS FOOT.

PIECE OF ROCK FELL ON EMPLYEE'S HAND.

EMPLOYEE WAS SHOVELING COAL ONTO CONVEYOR BELT AND HURT HIS LOWER BACK.

EE RAISEDUP & HIT HARD HAT ON ROOF.

EE WAS GOING TO HANG A BLOCK CURTAIN AND OTHER EE WAS TURNING A 3 WHEELER BUGGY. THAT EE DIDN'T KNOW EE WAS IN THE ARE AND DIDN'T SEE HIM. WHEN HE BACKED THE 3 WHEELER UP, EE'S LEFT LEG WAS CA UGHT BETWEEN THE SCOOP AND THE 3 WHEELER.

INJURED COMPLAINS OF WATER ON THE KNEE

EMPLOYEE WAS PUTTING A BOLT IN TOP & BOLT KICKE DOVER & HIT HIS WRIST.

EE WAS RIDING THE SCOOP IN, & THAE SCOOP HIT SOMETHING IN THE ROAD WAY [ROUGH PLACE IN ROADWAY]. WHEN THE SCOOP BOUNCED BACK DOWN, HIS BACK STARTED HURTING HIM.

EE WAS OPERATING BRIDGE CARRIER. DRAW ROCK FELL & HIT EE ON BACK.

THE HEAD OF THE ROOF BOLTER CAME DOWN ON TOP OF INJUREED KNEE PAD. EMPLOYEE WAS BOLTING AGAINST RIB. THE HEA DCAME DOWN FASTER THEN EXPECTED.

EMPLOYEE WAS PUTTING IN BELT STRUCTURE AND PULLED SOMETHING IN BACK.
