Showing all 7After replacing drum, shaft, bearings, and repairing belt splice, ee was testing adjusting and cleaning when accident occurred. Guard off, & belt energized, ee attempted to remove loose sand from edge of belt, in frt of drum, when glove came in contact with edge of drum, pulling him into contact with edge of drum with right bicep, inflicting open wound(skin), 5" long.
Employee was sitting normally in a chair inside scale house. The chair broke and he fell to the floor. He weight over a period of time apparently stressed the center shaft which allows the chair to pivot, thus causing a clean break of 1" diameter threaded shaft.
WHILE DISMOUNTING FRONT END LOADER THE OPERATOR LOST HIS BALANCE AND FELL APPROX 3 1/2 FEET LANDING ON LEFT ELBOW AND BREAKING A BONE AT THE ELBOW.
EE LOOKED AT INJURED FINGER AND FAINTED, STRIKING HEAD ON FLOOR.
EE STARTED TO CLEAN UP THE PICKUP AND WASH IT. HE BEGAN CLEANING THE FLOOR MAT WHEN HIS SAFETY GLASSES FELL OFF. HE THEN LEFT HIS GLASSES OFF AND TOOK A RAG WITH STEAM CLEANING SOAP TO CLEAN T HE FLOOR MAT. WHEN HE WENT TO PUT THE RAG BACK IN THE BUCKET SOME SOAP SPLASHED AND GOT HI HIS EYE SINCE HE HAD TAKEN HIS SAFETY GLASSES OFF.
MR RISTER VISITED HIS PHYSICIAN DR W H GRANDER OF SWANSEA SC NOT FEELING WELL DR GRANGER SENT MR RISTER FOR XRAY OF CHEST HE THE REFERRED MR RISTER TO DR RL GALPHIN OF COLUMBIA A PULMONARY SPE CIALIST DR GALPHIN DIAGNOSED THE AILMENT TO BE SILICOSIS
