Showing all 16Employee was setting up a conveyor when the landing leg fell on EE's foot causing a fractured bone.
Miner was cutting a conveyor belt with a blade for a patch. During the cut, the blade slipped off the belt due to the change in the amount of pressure they were applying and stabbed their thigh.
EE WAS REMOVING THE DOOR OF A SKIDSTEER. AS EE WAS DOING SO, THERE WAS A ZIP TIE THAT EE CUT TO ABRUPTLY WITH A BLADE AND CUT EE'S THUMB.
INSTALLING A SCREEN AT OUR WASH PLANT WITH ANOTHER EMPLOYEE AND AS THEY WERE SETTING IT INTO PLACE THE OTHER EMPLOYEE PUSHED ON THE SCREEN AND PINCHED THE HAND TO THE PLANT CAUSING A LACERATION THE REQUIRED STITCHES.
Employee was repositioning blocking under the crusher and another piece of blocking settled into the ground. As a result, they pinched fingers between the blocking and the landing gear. Employee received stitches on the left middle finger.
Conditions contributing are high step to a ladder of a rock truck. Injury appears to be an OD effecting the (L) Sciatica.
Employee was performing routine housekeeping; was removing dirt and rock from underneath the under-screen conveyor of the jaw. EE's shovel got caught in a return roller, which pulled EE's arm into the roller. EE freed self, but suffered major burns on right shoulder and arm.
Received Silicosis Diagnosis
I entered the machine, and my left foot pressed down on a foot control, which raise the bucket up into the air. As bucket lifted, it caught right arm and lifted me into the air. I was holding onto a side bar with my left hand, and when the bucket lifted it raised my whole body, and my left arm got pinched between the ROPS and the boom cross member.
Crusher Operator attempted to check the voltage with a volt meter at the electrical panel. After opening the panel door found existing lights not on. Picked up the volt meter to check and an arc flash occurred causing burns to the back of his hands.
EE was going into ramp to pick up bucket oversize rock, EE put the Cat in low gear during the initial movement into ramp whe he accidently touched left pedal with released clutch and loader started to free wheel and increased speed and hit pile of rock impact threw him forward hitting his head in corner where windshield and cab meet. Was not wearing seatbelt.
After viewing the material moving on infeed conveyor, he turned; right hand glove snagged by trough roller sucking right hand between roller and conveyor belt. The plant was in a test run after reconfiguring plant setup. There were no roller guards on that portion of the infeed conveyor. Damage to employee's hand was bruising. He was able to remove hand immediately.
WHILE REMOVING CONVEYOR SAND SPILLAGE AROUND TAIL PULLEYS, EE'S LEFT ARM WAS CRUSHED BETWEEN UNI-LOADER CAB & BUCKET.
EMPLOYEE WAS CLEANING OUT TAIL PULLEY ON CRUSHER WHEN ROCK STRUCK HIS HEAD. ALTHOUGH EMPLOYEE WAS ISSUED HARD HAT THAT INSTRUCTED TO ALWAYS WEARIT, HE FAILED TO WEAR HARD HAT AT TIME OF ACCIDE NT.
WHILE SEPARATING CRUSHER SCREENS & RESTACKING THEM, BENT OVER TO PULL THEM APART--FELT STRIAN IN LOWER BACK.
