ON 4 JULY 2001 EMPLOYEE WAS ADMITTED TO THE HOSPITAL. I WAS ADVISED BY HIS WIFE THAT HE WAS UNABLE TO RETURN TO WORK DUE TO SHORTNESS OF BREATH CAUSED BY BLOOD CLOTS IN HIS LUNGS (BLACK LUNG).
