EE WAS REMOVING ELECTRICAL CABLE FROM ONE FIELD TO TAKE TO ANOTHER. HE SHUT OFF 3 MAIN BREAKERS WHICH HE THOUGHT ENERGIZED AN ELECTRICAL RACK. HE USED THE CABLE CUTERS TO CUT AN ENERGIZED 480 VOLT CABLE ON THE ELECTRICAL RACK. THIS RACK WASENERGIZED FROM A DIFFERENT SOURCE. THE RACK WAS LABELED. HE SUFFERED ELECTRIAL SHOCK AND BACK MUSLE SPASMS. LOST 4 DAYS WORK.

EMPLOYEE WAS OPERATING A PUMPHOIST AND HAD PARKED UNDERNEATH SOME POWERLINES. HE WAS CARRYING SOME ELECTRICAL CABLE ON THE PUMP HOIST AND WAS ATTEMPTED TO RAISE THE MAST TO FACILITATE THE CABL E DROP-OFF. EMPLOYEE WAS AWARE OF THE PWERLINES BUT HE THOUGHT HE COUDL MISS THEM. THE MAST CONTACTED A POWERLINE AND CREATED A PHASE TO GROUDN CURRENT. EE SUFFERED BURNS TO HIS L HAND AND FT

EMPLOYEE WAS WELDING A BRACE TO AN I-BEAM WHEN HE WAS SUDDENLY SHOCKED ON THE LEFT ARM. HIS LEFT ARM WAS BEING USED AS SUPPORT AAINST A 3" PIPE. WAS UNABLE TO USE ARM FOR A SHORT PERIOD OF T IME.
