EMPLOYEE STATES WHILE CHECKING 1000 AMP BREAKER FOR PHASE ROTATION, VOLTAGE ACRED ACROSS THE TOP LUGS OF THE BREAKER. THE RESULTING FLASH CAUSED FIRST AND SECOND DEGREE BURNS ON FACE, BOTH AR MS AND BOTH HANDS.
EMPLOYEE STATE 2E GOT IN THE BAGGIN CHAIR TOBAG, AND WHEN HE TOUCHED THE BAGGING MACHINE HE RECIEVED A SHOCK. AS HE JUMPED FROM THE CHAIR HE TWISTED HIS LEFT KNEE WHICH WAS PREVIOUSLY INJURED ON 12-14-87. T2IS ACCIDENT DID NOT RESULT IN ANY LOST TIME UNTIL 1-19-88.
