EMPLOYEE WAS CHANGING OR CHECKING A FUSE IN THE DISCONNECT BOX AND APPARENTLY CAME IN CONTACT WITH THE MAIN FEED LINS TO THE FUSE BLOCK. EMPLOYEE HAD A SCREWDRIVER AND PLIERS IN HIS HAND AT TH E TIME AND THINKS HE STUCK THE TOOL IN THE BOX CAUSING THE SHORT BETWEEN THE TWO PHASES.
