A FAULTY TERMINAL ON ABTTERY IN THE PIECE OF EQUIPMENT.EMPLOYEE SHUT OFF PIECE OF EQUIPMENT,LATER TRIED TO START IT AND ENGINE WOUDN'T TURN OVER,HE THEN CALLED OVER HIS BUDDY TO HIT THE START BUTTON WHILE HE STOOD ON TOP OF BATTERY CABLE TO MAKE CONTACT.THIS CREATED SPARKS AND RESULTED IN BATTERY EXPLODING.
