THE EMPLOYEE WAS CONDUCTING AN INTEGRITY TEST ON WELL. WHILE EMPLOYEE WAS CHECKING FOR LEAKS, HE PLACED HIS RIGHT ARM ON THE TOP OF THE WELLHEAD AND APPLIED SOAPY WATER AROUND THE AREA WHERE W ELLHEAD ATTACHES TO THE CASING WITH HIS LEFT HAND. AT THIS TIME, THE WELLHEAD BLEW OFF STRIKING THE EMPLOYEE'S RIGHT ARM (ABOUT 120 LBS PSI).
