EE was preparing to erect a staging from the raise to a muck pile to bolt a raise up round when a rock approximately 12"x15"x 5" rolled against his foot and ankle causing significant bruising and swelling.

Showing all 8While operating LHD #2-6 in 4600 Up Ramp, rock came off rib, striking employee on right forearm.
Employee was drilling with a jackleg and standing close to the face. The face air slacked and rock struck the employee, causing two small lacerations which required sutures.
Rock burst while employee was bolting back. Employee fell against the footwall and hit right shoulder.
While cleaning bulkhead, rock fell from rib and struck employee in left wrist.
A rock outburst in the 4600 level Decline ramp caused the withdrawl of 2(two) miners from the work area during the rock bolting cycle of the mining cycle. One (1) miner sustained minor injuries.
Employee was knocked down from a rock burst. Employee said he was ok and returned to work the next shift. On 7/11/08 employee asked to see a doctor for his rt. elbow and was released full duty. On 7/30/08 employee revisited the doctor for his elbow and was placed on restricted duty and physical therapy treatments.
On 9/22/06, EE was installing rock bolts when a rock 6-8" in diameter fell from the Rib he was bolting, striking him on the head. The blow caused his jaws to come together, chipping two of his teeth. He was found to be uninjured & released. On 9/29/06, he had his teeth repaired, making this accident reportable.
