Employee was waiting for the bolt crew the finish spotting a bolt when they were struck in the back by a rock from the rib.

THE EE CLIMBED INTO A SHIELD WAITING FOR THE SHEARER TO PASS. WHEN THE SHEARER PASSED, THE EE CLIMBED OUT OF THE SHIELD, STEPPING ON THE NEXT SHIELD THAT HAD JUST PULLED, THE FLIPPER ON THE SHIELD TIP HAD NOT YET SWUNG OUT INTO POSITION. THE FACE SLOUGHED OUT ACROSS THE PANLINE AND BRETBY TROUGH, MATERIAL STRUCK EE IN LEFT LOWER LEG.

The employee was roof bolting in 6 left. As EE was in the process of installing the 2nd bolt in the last row, a portion of the rib rolled out behind EE. The rib rock struck the employee in the back, knocking EE into the bolter, pinning EE against the machine.

The employee was in the #6 entry when a portion of the right rib rolled out. The rib hit EE in the lower back, knocked EE to the ground, pinning EE's feet under the rock.

Employee was dragging a piece of curtain up #9 entry to ventilate the face while they bolted it when a rib measuring 11" thick X 29" wide X 27" high rolled out and struck the employee on the right ankle which resulted in a fracture to the right ankle.

While moving roof bolter cable, the rib sluffed off striking the employee.

While inspecting a work area and trying to pull a rib down, was struck on top of the right foot by the rib when EE couldn't move away fast enough. The material landed on top of EE's tarsal guard resulting in a closed fracture to a tarsal bone. Restricted to sitting work only until EE's next evaluation.

Employee was working on seal when rib section rolled out. Unable to avoid the rib and was struck in the lower back.

The CM operator backed the machine out of the face due to the machine's conveyor not working. The electricians had CM operator try to start the conveyor to determine the problem. A piece of rib of what would become the No. 2 face fell out onto the CM operator. The rib measured 53" long, 36" wide, and 20" thick.

The injured was operating the radio remote controlled continuous miner when the coal rib, at back, "popped" off the rib striking ee on the lower legs. The rib measured 9 feet long, by 5 to 7 inches thick and 6 feet high. This resulted in a fracture to right ankle.

Employee was driving a shuttle car from the continuous miner toward the feeder and as he rounded a tight corner a piece of rock came off of the rib and into the operator's deck mashing his left middle causing a laceration. The Laceration required ten sutures to close.

While conducting retreat mining on 6-Left Panel off B-4 Mains, a coal/rock outburst occurred inby break #35. The outburst involved the Mobile bridge operator who suffered multiple blunt trauma injuries involving the chest, right arm, and back.

While employee was trying to search for a broken tie rod end on the car he was operating, he was struck by a piece of rib rock that rolled down the rib striking his left ankle causing a fracture and dislocation.

While collaring a bolt hole with a jackleg drill, a slab of rock approx. 12" x 12" x 4" fell from the top of the rib and struck the miner on the back of his left hand, cutting through his glove and causing him to sustain a laceration on the back of his hand resulting in six stitches.

Operator was cutting a crosscut. As mining advanced, operator stepped away from rib mesh to look at cutting. At this point a rib fell and knocked operator to the ground. Operator received two broken ribs, cuts to the face, and a pulled shoulder.

EE was replacing a nozzle on a wash hose. Piece of coal (20"x8"x3"thick) slid down the rib and struck right hand. EE moved the wash hose valve next to the rib which was protected with wire mesh within 3' of the floor.EE was earing keviar gloves. Same knuckle was fractured of the job 3 months prior resulting in this re-fractured. No external marking or bruising of skin.

While the foreman was walking to the continuous miner, a piece of rib rock slid down the rib. When the foreman put his hand out, the rocks struck his right hand causing a laceration to his right index finger requiring sutures.

As the injured was cleaning the # 3 left cross cut at or near survey station 6781 a piece of rib rock rolled out and struck him in the left shoulder.

Injured was attempting to drop the miner cable from its hung position in order to move to the next cut. When he lifted the cable, a small piece of rock (2"x4") fell from the top of the rib mesh and struck him on the upper lip of his face near his nose. The edge of rock lacerated his upper lip requiring sutures.

EE was setting a base for a link and lock crib. A rib roll occurred and struck his left foot. The doctor put him on light duty for a few days. Vacation started a couple days later and he did not return. I have made several attempts to contact him and have not got a response from him.

Employee was putting away tools after changing bits on the continuous miner, when a rib fell and rolled onto employee and caused fractures to both ankles.

As the employee was removing the drill steel from a rib bolt hole, a piece of rib rock slid down the steel, striking his left hand. He suffered a fracture and laceration of his left index finger.

The foreman was watching production activities when a piece of rib rock fell striking him causing him to strike his nose against a mobile bridge unit. The employee continued working and sought medical treatment for his nose on 2/4/2011. The nose was determined to be fractured on that date.

The employee was working with other employees setting cribs at crosscut #64 along the longwall headgate, when the coal rib rolled off, hitting the employee and knocking him to the ground.

The continuous miner operator was standing between the second and third to the last row of roof bolts in the No. 1 entry (Gil-10 Headgate), completing a cut, when a rib roll occurred. The rib struck his left leg. The rib measured 6' x 6' x 0-24".

Employee was operating a continuous miner performing retreat mining, when a portion of the coal rib came loose and struck him on the back, knocking him down. He continued to work until 05/26/09 at which time he stopped due to the ongoing pain in his neck.

While tramming a roof bolter a piece of rib fell, striking the employee on the head and neck, causing a laceration to his forehead that requiried sutures. The rib material measured 3' by 2' and was 4" in thickness.

While shoveling a tailpiece, the employee was struck by a piece of rib rock on the head and neck. The rock measured 30" by 8" and was 1" to 4" in thickness. The employee continued working and started missing work on 4/13/2009.

The EE was in the process of advancing the face ventilation during the roof bolt cycle. He was behind the line curtain using his back & hips to hold the curtain away from the coal/rock rib. When he was bent over raking loose material to place on the curtain, a rib roll unexpectedly fell striking the injured on the arm & back of head. Cut to his forehead required sutures.

EMPLOYEE STATES HE WAS OPERATING THE CONTINUOUS MINER WHEN A RIB BLEW OUT, STRIKING EMPLOYEE IN THE MOUTH.

EMPLOYEE STATES HE WAS REACHING FOR MATERIALS WHEN A PIECE OF COAL RIB BLEW OUT, STRIKING EMPLOYEE ON LEFT SIDE RESULTING IN LEFT WRIST DISLOCATION AND LACERATIONS TO THE FACE AND ABOVE LEFT ELBOW.

EE was cleaning out to access the crawler take up on the miner, a small piece of rib/coal fell from the lower rib striking him in the lower leg. EE suffered a fracured tibia and a laceration to the leg.

He was tramming the Shearer to the tailgate when a bump occurred. A chunk of coal struck the left side of his Airstream helmet face shield, striking his cheek. He did not go to the doctor and continued to work. On 9/21/07 he was diagnosed with a fractured bone below his left eye, surgery is required.

Shoveling ribs along track entry under lifeline, rib rolled off hitting his right foot "causing swelling to right ankle." (additional narrative in quotes from MM 10/26/07 #9)

The inj EE was in process of assisting with the loading of a roll of 54" belt into a scoop bucket.As he was standing next to scoop, a rib (corner) 24"x40"x18" fell & struck the EE on rt leg.

The injured employee had walked up into the entry to check the positioning of the MRS he was two feet inby the corner standing next to the miner operator when a rib rock 62" x 12" x 14" popped off of the right rib striking him on the right shoulder and upper torso.

The injured stated he started to install first bolt in the bolt pattern when he heard a coal rib work loose. The injured started to exit from the front of the roof bolt machine. In the process the injured's hand, which was located on tray frame, was struck by the material.

He was standing outside of the shuttle car when a rib roll occurred and hit his leg against the deck of the car, resulting in a strain to the right knee. This occurred on the 003-0 section at the section feeder.

Employee was operating the Shearer on the longwall face when he heard someone calling on the face phone. He bent down to answer the face phone (with his back to the face). A slab of coal fell from the face landing in the face conveyor. The top section of the slab broke off and came into the walkway, striking him on the ankle.

Outby crew and longwall crew were setting roof and rib protection along stageloader and headgate. Injured person was between crib and meshed rib while topping crib. There was a bounce and employee was pushed into the crib.**Initial ER visit showed no injury. MRI taken 11/27/06 showed injury. Results received 11/30/06.**

Employee was laying drill pattern on top of coal. A rock fell from highwall. Employee saw it and tried to run but was hit in lower leg and foot.

Employee was bolting ribs. was struck by a 3'x 18' x 6' Rib.

Employee was checking the tail roller at the feeder breaker when a piece of coal rib (3" x 13" x 40" long), fell and hit the floor. The piece of coal then rolled forward and pushed the employee into the side of the feeder breaker, causing pain to his rib cage.

Employee was in the process of drilling a roof bolt hole when a rib rolled out and struck him. He continued to work and started missing work on 12/19/05.

EMPLOYEE STATES WHILE CUTTING COAL WITH SHEARER A PIECE OF COAL BURST FROM THE FACE, STRIKING HIS RIGHT FOOT.

Ee states after vent tube was hung he went to get back on right side platform of bolter. He grabbed hold of vent tube holder arm when a piece of rib fell hitting rib protector plate causing plate to rock forward which caught his finger.

THE INJURED WAS WALIKING DOWN THE SECTION ROADWAY WHEN A RIB 4' X 3' X 10" - 14" FELL AND STRUCK HIM ON THE LEFT LEG.

THE INJURY OCCURRED WHEN HE STARTED PRYING DOWN A LOOSE RIB WITH A PRY BAR. THE WHOLE RIB LINE APPROXIMATELY 25' LONG FELL OVER AND A PART OF THE RIB HIT HIM ON THE RIGHT FOOT CAUSING TWO SMAL L FRACTURES TO THE RIGHT FOOT.

EE WAS ON SHUTTLE CAR BEHIND MINER WHEN RIB ROLLED OUT, PART OF IT STRIKING HIM IN LEGS AND BACKJAMMING HIS LEG UP AGAINST HIS BACK.
